EB病毒抗体联合检测在鼻咽癌诊断中的应用评价

摘要 目的探讨EB病毒抗体在鼻咽癌诊断中的应用价值。方法采用间接免疫荧光法(ⅡF)测定124例患者(其中包括鼻咽癌53例、鼻息肉26例、鼻咽炎31例、颈部淋巴结炎14例)和正常体检40例血清中4项EB病毒抗体。结果EBV VCA-IgA与EBV-EA-IgG在鼻咽癌组与鼻息肉、鼻咽炎和颈部淋巴结炎组及正常对照组的检测阳性率有非常显著差异(P<0.01);EBV- CA-IgA在鼻咽癌患者的检测阳性率71.7%。以VCA-IgA和EA-IgG两项指标进行平行试验,其敏感性(Sen)为86.1%。以VCA-IgA和EA-IgG两项指标进行序列试验,特异性(Sep)为99.5%。结论鼻咽癌病人的EBV-CA-IgA、EBV-EA-IgG阳性率高、特异性强,可作为鼻咽癌血清学诊断指标。EBV-CA-IgA与EBV-EA-IgG两种抗体同时检测可提高鼻咽癌血清学诊断敏感性和特意性。鼻咽癌以鳞状细胞癌较多,其分化程度越高,EBV-CA-IgA抗体滴度越低。 Objective: The application value of EBV antibodies were studied in the diagnosis of carcinoma of nasopharynx. Method: The four EBV antibodies in the 124 patients serum ( including 53 cases of carcinoma of nasopharynx , 26 cases of nasal polyp, 31 cases of nasopharyngitis and 14 cases of cervical lymphadenitis) and 40 cases of regular examination persons were detected with indirect immunofluorescence(IIF). Result: The positive detection rate of EBV-CA-IgA and EBV-EA-IgG were significantly different between carcinoma of nasopharynx group and nasal polyp, nasopharyngitis and cervical lymphadenitis group, so as with normal control group(P〈0.01). The positive detection rate of EBV-CA-IgA was 71.7% in carcinoma of nasopharynx patients. In the parallel test of VCA-IgA and EA- IgG, the sensitivity of the two index was 86.1%, and in sequence test the speciality were 99.5% in carcinoma of nasopharynx patients . Conclusion: For the EBV-CA-IgA and EBV-EA-IgG have high positive rate and reliable specificity in carcinoma of nasopharynx patients, they can be used as diagonostic serum index for carcinoma of nasopharynx. With the combination detection of EBV-CA-IgA and EBV-EA-igG, the sensitivity and speciality of carcinoma of nasopharynx serum diagnosis can be raised. Most carcinoma of nasopharynx belongs to Cutis Squamous Cell Carcinoma, and the higher differentiation degree it is, the lower EBV-CA-IgA titer will be.
