胸腔镜下食管癌手术对术后早期肺功能影响的临床研究 被引量:32

摘要 目的探讨胸腔镜下行食管癌手术后早期对呼吸功能的影响。方法2005年3~7月入院治疗的食管癌患者68例,随机分成胸腔镜组(实验组34例)和常规开胸组(对照组34例),检测术前及术后1、2周肺活量(VC),1s呼气肺活量[FEV1.0],最大通气量(MVV)实际值占预计值的百分比以及术后1、2、3、5、7和10dPaO2,进行统计学分析。结果两组术后肺功能降低有显著意义,胸腔镜组肺功能损害较常规组显著减轻。结论胸腔镜下行食管癌手术对呼吸功能影响较小,适用于肺功能较差的病人。 [Objective] To observe the influence of the early pulmonary function after video-assisted thoracopic surgery, (VATS). [Methods] 68 inpatients on March 2005 to the duration of July 2005 were prospectively randomized into two groups, 34 cases received video-assisted thoracopic surgery (test group); 34 cases received routine open thoractomies (control group). Detecting VC, FEVL0, MVV% (Detected value/estimated value) before and after operation on 1, 2 weeks, and PaO2 of 1, 2, 3, 5, 7 and 10 days, carry out statistics analysis. [Results] Reducing that pulmonary function of two groups after surgery had statistics meaning, the VATS group pulmonary function damaged ferther less than routine open thoractomies group. [Conclusion] VATS has less influence on pulmonary function than the rountine open thoractomies surgery, fired for the patients who have bad pulmonary function.
