增强减影MRI技术诊断椎体转移瘤 被引量:2

Contrast-enhanced digital subtraction MRI for diagnosis of vertebral metastatic tumors

摘要 目的评价增强减影 MRI 在椎体转移瘤诊断中的应用价值。方法对30例44个椎体转移瘤进行常规 MR 序列扫描,同时使用对比增强剂钆喷替酸葡甲胺进行增强显影,并对增强前后的 T_1WI 进行减影处理。通过对椎体肿瘤影像的清晰度、信号强度比(SIR)和噪声比(NR)的比较,评价增强减影 MRI 在椎体转移瘤上的诊断价值。结果增强减影 MRI 及增强 T_1、SE T_1、T_2、反转恢复序列(STIR)图像的 SIR 值分别为2.93、1.15、1.16、1.26、1.69。增强减影 MRI 及增强 T_1、SE T_1、SET_2、STIR 图像的 NR 值分别为0.98、5.25、3.44、4.56、23.32。增强减影 MR 图像的 SIR 和 NR 较常规MRI 图像差异均有统计学意义(P<0.05),其信号对比度及图像质量均较好。结论与常规 MR 序列图像相比,增强减影 MRI 对椎体转移瘤病灶的细节显示得更清楚,病灶与背景的对比度更明显,图像质量更清晰,在椎体转移瘤的临床诊断上有重要的应用价值,可以在增强显影后常规应用。 Objective To evaluate the diagnostic value of contrast-enhanced digital subtraction MRI in vertebral metastatic tumors. Methods Forty-four vertebral metastatic tumors in thirty patients were scanned by routine MRI including SE T1WI, SE T2WI, STIR and enhanced T1WI with an injection of Gd-DTPA(0. 1mmol/kg). Digital subtraction was performed between pre-contrast and enhanced T1 weighted images. All the images of vertebral malignant tumors were evaluated by means of signal intensity ratio (SIR) and nose ratio (NR). The quality of images was also evaluated by comparing subtraction MRI with routine MRI. Results SIR and NR of subtraction MRI was 2.93, 0. 98 respectively. SIR of routine MRI (enhanced T1WI,SE T1WI,SE T2WI,STIR) was as follows: 1.15,1.16,1.26,1.69. While NR of those was 5.25,3.44,4. 56,23.32 respectively. SIR and NR of subtraction MRI images had significant statistical differences from those of routine MRI images ( P 〈 0. 05 ) . Conclusion Contrast-enhanced digital subtraction MRI images present clearer details, more intensive contrast, and better quality of images than routine MRI imagos. It can be performed after enhanced MRI routinely to diagnose vertebral metastatic tumors.
