摘要 目的 探讨PET-CT神经导航脑胶质瘤切除术的临床价值。方法 两例考虑为胶质瘤的患者,以脑CT和MRI及PET-CT检查进行术前评估,并在PET-CT导航下行开颅肿瘤切除术。结果 常规脑CT和MRI检查均不能分辨两例肿瘤的确切部位、范围和与周围水肿的关系;PET-CT显示肿瘤的位置和边界清晰,并能揭示肿瘤增殖的不均一性,为神经导航提供了准确的肿瘤信息。结论 对于常规影像学不能明确界限的胶质瘤,基于PET-CT的神经导航手术对提高肿瘤全切率和揭示胶质瘤生物学特性有较高的临床和研究价值。 Objective To evaluate the application of positron emission tomography/computerized tomography (PET/CT) neuronavigation in surgery for gliomas. Methods Two patients with a provisional diagnosis of ghoma were investigated with CT, MRI, and PET/CT, in order to evaluate the tumors before operation. The tumors were then resected under the guidance of PET/CT neuronavigation. Result CT and MRI were not able to distinguish the exact border of the tumor and their relation to the surrounding edema tissue. PET/CT however showed the location, extent and heterogeneity of the tumors clearly, and provided exact data to neuronavigation system. Operation using neuronavigation was then carried out, based on these data. Conclusions In the management of gliomas in which CT and MR imaging alone fail to show the boundaries of the tumor clearly, PET/CT neuronavigation methods appear to have great value in improving the completeness of the resection of tumor, and also in indicating its biological characteristic.
