摘要
[目的]结合近年来不断出现和完善的各种新技术,探讨和设计一种新的手术方式用于减少传统手术方式的并发症,使腰椎后路手术更快,更好。[方法]腰椎全椎板覆盖式手术。[结果]20例不同类型的腰椎退行性病变的病人用该术式进行的手术证明安全、有效。[结论]这种手术方式避免了传统全椎板切除和椎板开窗式手术主要的缺陷,又保留了这两种手术各自的优点。术中具有显露时间短,范围大,出血少,术野清晰,有利于术者有效、彻底地进行神经根减压,椎间融合和椎体复位等优点。全椎板覆盖后,辅以椎弓根钉固定系统的帮助,术后不但可以完整保留了后路骨性结构对椎管及神经根的保护,减小了手术后瘢痕黏连压迫,而且增加了小关节突融合的可靠性。
[Objective] To explore and design fewer time-consuming and better method in posterior lumbar approach. [ Method] Totally Imninectomy and reimplantation were preformed in 20 cases of lumbar degeneration patients. [ Result ] Laminograftomy was proved to be safe and efficacy in 20 lumbar degenerative cases in various kinds. [ Conclusion ] This technique, on one hand, doesn＇t share weak points of total or semi laminectomy, on the other hand, maintains strong points of mentioned teclmiqueo It can save time, offer broad exposure with fewer blood loss and standard and clear operational field. These allow surgeons effective and full decompression of nerve root and other manipulation such as, intervertebral fusion and vertebral reduction. As a result of laminograftomy and pedicel screw system, it can preserve the protection from posterior column bony structure for spinal canal and nerve root, and prevent scars. What＇s more, it enhance the reliability of facet fusion.
