摘要
目的:观察关节镜下微骨折技术(Microfracture)修复膝关节软骨全层缺损的疗效。方法:回顾性分析2002年1月~2005年7月北京大学运动医学研究所收治的43例膝关节软骨全层缺损患者的情况,平均随访15.9±8.8个月(6~47个月)。采用Tegner、Meyers、Lysholm评分作为疗效判定标准。结果:微骨折技术修复膝关节软骨全层缺损总有效率为87.0%,其中,疗效优27个膝关节(58.7%),疗效良13个膝关节(28.3%),疗效差3个膝关节(6.5%)。平均Tegner评分从1.5±1.3提高到4.8±1.5(P〈0.01);平均Meyers评分从9.9±1.9提高到16.4±2.0(P〈0.01);平均Lysholm评分从41.8±12.5提高到82.7±12.6(P〈0.01)。手术前后疗效比较有统计学差异(P〈0.01)。微骨折术疗效与患者的性别、半月板损伤的相关性不明显(P〉0.05),但与患者的年龄、外伤、软骨缺损面积存在相关性(P〈0.05)。结论:关节镜下微骨折技术修复膝关节软骨全层缺损操作简单、安全,微创,疗效可靠,是较为实用的关节软骨修复技术。
Objective To study the clinical effect of repaired full- thickness articular chondral defects of the knee by arthroscopic microfracture technique. Methods From January 2002 to July 2005, forty - three patients who suffered from full - thickness articular cartilage defects and treated with arthroscopic microfracture technique were studied retrospectively. Patients were followed up for an average of 15.9 ± 8.8 months (range: 6 - 47months). Tegner Activity level scale, Meyers score, Lysholm score were used for the evaluation before and after operation. Results Total effective rate of clinical outcome was 87.0%, including 27 cases with excellent result (58.7%) and 13 cases with good result (28.3%). Meanwhile,there were 3 cases showed bad result (6.5%). Mean Tegner Activity level scale improved from 1.5 ±1.3 to 4.8± 1.5 ( P 〈 0.01). Mean Meyers score improved from 9.9±1.9 to 16.4±2.0 (P 〈 0.01) and mean Lysholm score improved from 41.8±12.5 to 82.7±12.6 (P 〈 0.01). No significant correlation was noted between Lysholm improvement and gender or menisci tear( P 〉 0.05). However, there existed a negative correlation between Lysholm improvement and age, trauma, defect area ( P 〈 0.05 ). Conclusion Arthroscopic microfracture technique was simple, safety, micro - invasive and effective, appeared to be a practical surgical technique for the treatment of articular chondral lesions of the knee.
