MEIA检测低水平血清HBsAg及结果分析

Determination of Low Level HBsAg in Serum by Microparticle Enzyme Immunoassay and Analysis of Results

摘要 目的 了解低水平血清乙肝病毒表面抗原(HBsAg)的人群分布及其相关乙肝病毒血清标志物模式特征。方法 微粒子酶免疫分析技术(microparticle enzyme immunoassay,MEIA)测定8089例非肝炎病区住院患者血清HBsAg及其表面抗体(抗-HBs)、乙肝病毒e抗原及其e抗体(HBeAg、抗-HBe)和乙肝病毒核心抗体(抗-HBc);根据定值参比血清和样本的HBsAg荧光速率值/阴性对照荧光速率值之比值(S/N值),并结合中和试验结果,确定浓度在5μg/L以下的HBsAg阳性例数,并分析相关乙肝病毒(HBV)血清标志物模式。结果 共检出HBsAg阳性816例,HBsAg浓度在5μg/L以下的有189例,占总数的2.34%,占HBsAg阳性人群的23.16%,其中,HBsAg浓度在1μg/L以下的有84例(44.04%);1~2μg/L的有33例(17.5%);2~5μg/L的有72例(38.10%)。对上述低水平HBsAg人群的5项HBV血清标志物检测获得8种模式,以“HBsAg、抗-HBc、抗-HBe阳性”,“HBsAg和抗-HBs阴性”模式为主(74.60%);累计HB—sAg和抗-HBc同时阳性者占94.17%;HBsAg与HBs同时阳性只出现在HBsAg 1μg/L以下人群中(6.45%)。结论 低水平血清HBsAg人群不容忽视,提高HBsAg检测灵敏度有重要意义;同时检测相关HBV血清标志物,对于确定上述人群有帮助。 Objective To investigate the distribution of the population with low level HBsAg in serum and the characteristics of their HBV serologic markers. Methods Serum HBsAg, surface - antibody (anti - HBs), hepatitis e antigen and antibody (HBeAg and anti- HBe), and hepatitis core antibody(anti- HBc) in 8,089 inpatients in the non- hepatitis endemic area were examined by method of micro- particle enzyme immunoassay (MEIA). According to the ratio of HBsAg fluorescence velocity value of the infected vs. the negative control serum value, and referring the results of neutralization test, we determined the cases with HBsAg positive under the concentration of 5μg/L, and analyzed the marker mode of HBV positive sera. Results Altogether 189 patients with HBsAg low than 5ug/L (2.34% of all 8,089 patients and 23.16% of 816 HBsAg positive patients) included 84(44.04% ), 33(17.5% ), and 72 (38.10%) with HBsAg less than lug/L, 1-2ug/L, and 2- 5ug/L respectively. Eight modes were obtained from the .5 serum markers of low level HBsAg among the above- mentioned population, the main modes included the "HBsAg, anti- HBc, Anti- HBe positive ", and the "HBsAg and anti- HBs negative". The patients with both HBsAg and Anti - HBc positive were 94.7 %. The patients with both HBsAg and HBs positive only appeared in the population (6.45%) with HBsAg 1μg/L, orbelow. Conclusions The population with low level HBsAg in serum should not be neglected and it is important to improve the test sensitivity of HBsAg and to detect the other HBV serologic markers.
