摘要
目的:了解良性前列腺增生症(BPH)并急性尿潴留(AUR)患者前列腺特异性抗原(PSA)水平增高的相关因素。方法:回顾性分析123例BPH并发AUR(A组)及137例未并发AUR(B组)患者的相关资料。结果:A组患者年龄(73.30±9.59)岁,前列腺体积(PV)(58.18±36.66)ml,血清PSA(10.39±10.78)μg/L;B组患者年龄(72.04±9.19)岁,PV(43.51±23.45)ml,血清PSA(4.35±4.96)μg/L。A、B组间PV和血清PSA差异均有显著的统计学意义(P均<0.01)。3个不同PV范围(PV<30ml,PV=30～50ml,PV>50ml)A、B组间前列腺体积和血清PSA进行比较,血清PSA差异均有统计学意义(P<0.05);PV<30ml及PV=30～50ml,PV差异无统计学意义(P>0.05),PV>50ml,PV差异有统计学意义(P<0.05)。血清PSA4～10ng/ml之间,A组40例,B组37例,两组比较无统计学意义(P>0.05)。血清PSA>10ng/ml,A组46例,而B组11例,两组比较差异有显著统计学意义(P<0.01)。A组患者血清PSA与前列腺体积(r=0.492,P<0.01)呈显著正相关,与年龄呈正相关(r=0.186,P<0.05)。结论:影响良性前列腺增生症并急性尿潴留患者血清PSA的因素是多方面的,除了前列腺体积和年龄等因素外,还应考虑到急性尿潴留本身的影响。
To study the correlated factors of the serum prostate specific antigen （PSA） in the benign prostatic hyperplasia（BPH） patients with acute urinary retention （AUR）.Menthods：A total of 260 cases of men with benign prostate hyperplasia were retrospectively studied,and patients with prostatic cancer were excluded.The age,the serum prostate specific antigen and acute urinary retention were recorded,and prostate volume were measured by transabdominal uhrasonography.These variables were correlated with the results from AUR of BPH study. There were 123 cases of benign prostate hyperplasia with acute urinary retention and 137 cases without acute urinary retention.Results：In the group of acute urinary retention, the mean age was （73.30±9.59） years,the mean prostatic volume was （58.18±36.66）ml, the mean the serum PSA （10.39±10.78）p,g/L;In the group of without acute urinary retention, the mean age was （72.04±9.19） years,the mean prostate volume was （43.51±23.45）ml,the mean the serum PSA （4.35±4.96）μg/L.There were statistically significant differences between the two groups in prostate volume and PSA （all P〈0.01）.In the different prostate volumes （PV〈30ml,PV= 30-50 ml （all P〈0.05）,PV〉50 ml）,there were statistically significant differences between the two groups in the serum PSA and prostate volume（PV〉50 ml）, and in PV〈30 ml and PV=30-50 ml,there was no statistically significant difference between the two group in prostate volume （P〉0.05）.There was no obvious difference incidence of AUR between the two group in the serum PSA in 4-10 μg/L （P〉0.05）,and statistically significant different incidence of AUR between the two group in the serum PSA〉10μg/L （P〈0.05）.In the benign prostatic hyperpiasia with acute urinary retention patients,the serum PSA was positively correlated to prostate volume and the age（P〈0.01）, prostate volume was positively correlated to the age（P〈0.05）.Conclusion：Among these related factors ＇affecting serum PSA,not only prostate volume and age,but,also acute urinary retention itself plays a role in AUR of BPH.
