腔内激光治疗下肢静脉曲张232例临床分析 被引量:13

摘要 目的 探讨腔内激光治疗术(EVLT)治疗下肢静脉曲张的临床疗效及并发症。方法对2003年8月~2005年4月间应用EVLT治疗的232例患者384条肢体的临床资料进行回顾性的分析和总结。结果单纯采用EVLT治疗的72例102条肢体,22条患肢有大隐静脉的分支静脉复发及残留,复发率为21.57%。采用高位结扎加EVLT治疗的160例282条肢体有4条肢体术后复发,复发率为1.42%。术后常见并发症有瘀血(22/384,5.73%)、静脉炎反应(61/384,15.89%)、灼伤(15/384,3.91%)、隐神经损伤(721384,18.75%)、股静脉血栓形成(1/384,0.26%)。结论 采用大隐静脉高位结扎结合EVLT能减低复发率,提高临床疗效。掌握其适应证和规范操作能避免并发症的发生。 Objective To evaluate the clinical effect and complication of endovenous laser treatment (EVLT) on varicose veins of lower extremity. Methods The clinical data of 232 cases with varicose veins of lower extremity treated by EVLT from August 2003 to April 2005 were summarize retrospectively. Results 102 limbs of 72 patients were treated by EVLT only,the recurrence rate was 21.57% (22/102) ,282 limbs of 160 patients were treated by EV- LT combing with high ligation of great saphenous vein,the recurrence rate was 1.42%(4/282) .The complications after operation was stagnant blood (22/384,5.73 % ), phlebitis(61/384, 15.89 % ), ambnstion( 15/384,3.91% ), injury of saphenons nerve (72/384, 18.75 % ) and femoral vein thrombosis ( 1/384,0.26 % ). Conclusion Combining both high hgation of great saphenons vein and EVLT may reduce recurrence rate and improve curative effect. The complications can be avoided when grasping the indication and operating correctly.
