股内侧肌和腓肠肌疲劳后反应时的变化及郑氏按摩手法的干预效应 被引量:4

摘要 目的:观察股内侧肌和腓肠肌疲劳后反应时的变化及应用郑怀贤教授创立的郑氏按摩手法对这种变化的影响。方法:于2005-05/06选择成都体育学院运动医学系健康男性大学生30人,随机抽签法分为3组:单纯训练组,按摩组,理疗组,每组10人。下肢大强度运动方式:①功率自行车蹬车20min,阻力3档,速度20km/h。②优势腿单脚跳楼梯22级×5组。组间休息1min。③优势腿在CYBEX等动练习器上做膝关节屈伸练习2组,60次/组,角速度为180(°)/s,两组间休息10min。每种运动方式后休息5min。运动结束后,立即进行恢复性治疗。按摩组以郑氏按摩手法重点放松股四头肌、腘绳肌和小腿三头肌15min。理疗组采用自制活血化瘀中药液(红花、川芎、木香、血竭等14种中药组成)和CLJL-3多功能经络治疗仪作中药离子导入,电极安放位置为血海-梁丘,阴-阳陵泉,悬钟-太冲共3对,时间20min。单纯训练组不做特殊处理,自然休息恢复。于运动前、运动后即刻和恢复4h采用肌电图仪分别记录受试者股内侧肌和腓肠肌的肌肉反应时,包括脊髓反射、中间反射和自主肌肉活动肌肉反应时。结果:各组大学生均完成训练和各时段指标测试,全部进入结果分析。①单纯训练组运动后即刻股内侧肌和腓肠肌的脊髓反射时、中间反射时、自主肌肉活动时均比运动前显著延长,差异有显著性意义(P<0.01)Z恢复4h后股内侧肌中间反射时和腓肠肌脊髓反射时仍比运动前显著延长,差异有显著性意义(P<0.01)。②理疗组运动后即刻股内侧肌和腓肠肌的脊髓反射时、中间反射时、自主肌肉活动时均比运动前显著延长,差异有显著性意义(P<0.01)Z恢复4h后股内侧肌脊髓反射时和腓肠肌中间反射时仍比运动前显著延长,差异有显著性意义(P<0.01)。③按摩组运动后即刻股内侧肌和腓肠肌的脊髓反射时、中间反射时、自主肌肉活动时均比运动前显著延长,差异有显著性意义(P<0.01)Z恢复4h后股内侧肌和腓肠肌的脊髓反射时、中间反射时、自主肌肉活动时均与运动前接近,恢复正常,差异无显著性意义(P>0.05)。结论:运动后即刻股内侧肌和腓肠肌的反应时显著性延长,是疲劳后膝关节受伤增加的主要原因之一。郑氏按摩手法可促进股内侧肌和腓肠肌反应时的恢复。 AIM: To observe the changes of reaction time of muscle vastus medialis and gustrocnemius after fatigue and the interventional effects of Zheng's massage initiated by Zheng Huaixian on the changes. METHODS: Thirty healthy male students from the Department of Sports Medicine, Chengdu Physical Education College from May to June 2005 were selected randomly divided into 3 groups: training group, massage group and physical therapy group with 10 students in each group. Heavy intensified sports of lower limbs was adopted as follows: ①To cycle for 20 minutes with a speed of 20 km per hour and a resistance of third level; ② To jump with one leg only for 22 steps of 5 groups with 1 minute's rest among the intervals; ③To bend and extend the knee joint with CYBEX for 60 times of 2 groups with an angular velocity of 180°per second and a 10 minutes' rest among the interval. After each way of sport the students rested for 5 minutes. The recovery treatment was given right after the sports. The massage group was asked to relax quadriceps femoris, hamstring and triceps muscle of calf with Zheng's massage for 15 minutes. The physical therapy group was treated by ion induction of self-made mixed liquid (composed of 14 different Chinese medicines such as safflower, rhizoma, aucklandiae, dragon's blood) through CLJL-3 multifunction main and collateral channels therapy instrument. The electrodes were placed symmetrically on Xuehai (SP10) and Liangqiu (ST34), Yinlingquan (SP9) and Yanglingquan (GB34), and Xuanzhong (GB39) and Taichong (LR3) for 20 minutes. The training group was not given any treatment, just relaxed. The electromyograph was used to record and test the response time of the examinees on spinal reflex, intermediate reflex and voluntary muscle activities of muscle vastus medialis and gastrocnemius before, right after the exercise and 4 hours after recovery. RESULTS: All the students completed the training and test at each stage and entered the result analysis. ①For the training group, there were significant differences in response time of spinal reflex, intermediate reflex and voluntary muscle activities of muscle vastus medialis and gastrocnemius between fore-and-after exercise (P 〈 0.01). Meanwhile, the intermediate reflex of muscle vastus medialis and the spinal reflex of gastrocnemius after 4 hours recovery were obviously prolonged compared with that before exercise, which had significant differences (P 〈 0.01). ② For the physical therapy group, the reaction time on spinal reflex, intermediate reflex and voluntary muscle activities right after exercises was significantly prolonged compared with that before exercise, but had no significant difference (P 〈 0.01); After relaxing for 4 hours, the intermediate reflex time of muscle vastus medialis and spinal reflex of muscle gastrocnemius was still longer than that before exercise, which had significant difference (P 〈 0.01). ③For the massage group, there were significant differences in response time of spinal reflex, intermediate reflex and voluntary muscle activities of muscle vastus medialis and gastrocnemius between before and right after exercise (P 〈 0.01).Meanwhile, the intermediate reflex of muscle vastus medialis and the spinal reflex of gastrocnemius after 4 hours recovery were similar to that before exercise, which had no significant difference (P 〉 0.05). CONCLUSION: The prolonging of reflex time of muscle vastus medialis and gastrocnemius is one of the main reasons for the increase of knee joint injury after fatigue. Zheng's massage can help promote the recovery of the reaction time of muscle vastus medialis and gastroenemius.
