细菌耐药突变选择窗理论与抗菌药物合理应用 被引量:31

摘要 目的总结防细菌耐药突变株选择浓度(MPC)和突变选择窗(MSW)理论的研究概况,提出抗菌药物合理应用的新策略。方法检索查阅了近5年的30多篇有关文献。结果血清或组织液药物浓度只有高于MIC并低于MPC,处于MSW内时,才导致耐药突变体的选择性扩增,即使临床治疗成功率很高;低于MIC时,治疗无效也不会导致细菌耐药突变体的富集;高于MPC时,细菌要生长须同时具备两种或以上突变,因而不仅治疗成功,也很难出现耐药突变体的选择性扩增。结论基于MSW理论治疗策略的原则是,当血或组织药物浓度高于MPC时,可采用单药治疗;不能达到MPC时,需不同作用机制的药物联合治疗,以关闭MSW,从而在控制感染的同时防止细菌耐药的发生发展。
