摘要
目的探讨小鱼际皮瓣的显微外科解剖学基础及游离移植修复指腹缺损的方法。方法10例废弃手标本,10倍手术显微镜下观察该皮瓣血管神经的显微结构特点,并应用该皮瓣游离移植修复指腹缺损(7只手)。结果显微解剖学观察废弃手10例标本及患者7例标本小鱼际区域血供有4个来源,其中尺动脉有一特殊分支进入小指展肌。7只手游离皮瓣全部成活。术后随访6—12个月,手外形及功能恢复满意。结论小鱼际皮瓣血管蒂的长度口径符合显微外科的要求,可用于游离移植;皮瓣结构、血管口径与指部相当,是修复指腹软组织缺损较理想的方法。
Objective To study the microsurgery anatomical basis of the hypothenar flap and to provide method for repairing ventral soft tissue defect of the finger through free transplantation of the flap. Methods 10 fresh abandoned hands were observed the origins, distributions and anastomosis of the blood vessels and nerves under the operative microscope. The flaps were freely transplanted to repair ventral defect of the finger in 7 cases. Results The blood of the hypothenar district of 10 specimens and 7 patients provides four sources, and the foot artery contains a special branch enter the little finger exhibition muscle. 7 cases free flaps survived. All cases were followed up 6 - 12 months after operation, and the appearance and function of the hands recovered well. Conclusions The length and caliber of the vessel pedicle of the hypothenar flap fit the demand of the microsurgical anastomosis and can be applied in freely transplantation ; the flap's construction and vessel caliber are similar to the finger and suitable for repairing ventral defeet of the finger.
