摘要
目的探讨短期胰岛素强化治疗对胰岛素抵抗和代谢紊乱的影响。方法将36例初诊2型糖尿病合并代谢综合征患者随机分为两组,在基础治疗的前提下,分别使用短期胰岛素强化治疗和口服降糖药治疗30d,治疗前后观察空腹和餐后2h血糖、空腹和餐后2h胰岛素、胰岛素敏感指数、血胆固醇、甘油三酯、高密度脂蛋白、体重、体重指数、腰围、腰臀比、收缩压、舒张压的变化。结果胰岛素强化治疗后空腹血糖、餐后2h血糖、餐后2h胰岛素、血胆固醇、甘油三酯、收缩压、舒张压,较治疗前明显降低,高密度脂蛋白和胰岛素敏感指数较治疗前明显升高,P<0.05;口服药对照组治疗后空腹血糖、餐后2h血糖、收缩压、舒张压较治疗前明显降低,P<0.05;胰岛素强化治疗组与口服药对照组治疗后相比,空腹血糖、餐后2h血糖、餐后2h胰岛素明显降低,胰岛素敏感指数明显升高,P<0.05。结论短期强化胰岛素治疗2型糖尿病合并代谢综合征可以更好的纠正糖脂代谢紊乱,减轻胰岛素抵抗。
Objective To investigate the effects of short term intensive insulin treatment for newly diagnosed type 2 diabetic mellitus combined with metabolic syndrome. Methods Thirty-six cases of newly diagnosed tape 2 diabetic mellitus combined with metabolic syndrome were divided into two groups randomly, intensive insulin treatment group was treated by insulin ,4/d,ih; the control group was treated by oral antidiabetic drug. Prior and after treatment, determined the levels of serum insulin, blood glucose, insulin sensitivity index, blood cholesterin, triglyerid, high density lipoprotein, body weight, body mass index, waistline, waist-to-hip ratio, systolic blood pressure, and diastolic blood pressure. Results After short term intensive insulin treatment, the levels of fast blood glucose, post blood glucose, 2-hours serum insulin, blood cholesterin, triglyerid, blood pressure were significantly decreased, high density lipoprotein and insulin sensitivity index increased obviously. After oral antidiabetic drug treatment, the levels of fast blood glucose, post blood glucose and blood pressure were lower than before; the levels of fast blood glucose, post blood glucose, 2 hours serum insulin in intensive insulin treatment group were lower than oral antidiabetic drug treatment group;and the insulin sensitivity index in intensive insulin treatment group was higher than the other. Conclusion Short term intensive insulin treatment can retrieve disorders of glycometabolism and lipid metabolism, is better than oral antidiabetic drug treatment. And it can lessen insulin resistance.
