摘要
目的 研究高海拔地区不同体重指数的2型糖尿病患者心脏结构和功能的变化特点。方法 选择2型糖尿病患者73例,健康体检者27例,根据体重指数(BMI)分为三组(BMI 18.5~22.9为1组、BMI23~24.9为2组、BMI≥25为3组),应用彩色多普勒超声心动图仪对比分析各组间心脏结构和功能的检查结果。结果 各组心脏结构参数比较:第3组心脏左房前后径、左心室舒张末期内径、左心室收缩末期内径、室间隔厚度、左室后壁厚度大于2组(P〈0.05),明显大于1组(P〈0.01)。各组心脏功能参数比较:第3组舒张早期血流充盈峰值流速E、E/A比值较2组降低(P〈0.05),明显低于1组(P〈0.01),而舒张晚期血流充盈峰值流速A3组高于2组及1组(P〈0.05),收缩及舒张功能异常率三组同健康组比较差异有显著性(P〈0.01)。结论 2型糖尿病患者随着BMI的增加心脏结构发生改变,心脏功能随之降低。
Objective To compare the changes of cardiac structure and function in different body mass index ( BMI) with type 2 diabetes mellitus ( T2DM) in high altitude area. Methods Seventy three T2DM were divided by the body mass index(BMI) into 3 groups. Color Doppler Echocardiography was performed in all of the cases, to compare and analyze the results of cardiac structure and function. Results LA, LVEDD, LVEDS, IVS, LVPW of group 3 were significantly larger than group 1 ( P 〈0.01 ) and group 2 ( P 〈0.05 ). group 3 of the E wave velocity and E / A ratio were significantly lower than those of group 2 ( P 〈 0.05 ) and group 1 ( P 〈 0.01 ). A wave velocity of group 3 was higher than group 2 and 1 ( P 〈 0.05 ) , There was significant changes in cardiac systolic function and filling dysfunction in group of T2DM than normal group ( P 〈 0.01 ). Conclusion The patients of T2DM can lead to change of cardiac structure and function which dependent with BMI degree.
