摘要
目的研究严重后巩膜破裂伤的手术治疗。方法回顾性分析2004年1月-2005年10月眼后段严重破裂伤患者19例19眼，伤口长度10mm以上，眼内容物部分脱出或嵌顿。均清创缝合，一期或二期玻璃体切除及硅油填充术。随访3个月-2年。结果眼部伴随病变包括眼内积血、视网膜下出血、脉络膜出血性脱离、视网膜脱离、眼内容脱出、视网膜血管闭塞、视神经损伤、虹膜缺失或根部断离、晶状体脱位等。术前8眼视力光感可疑，11眼无光感。术后视力4眼无光感；2眼光感；〈0．01者2眼；0．01≤最佳矫正视力〈0．05者5眼；0．05≤最佳矫正视力〈0．3者3眼；最佳矫正视力〉0．3者3眼。结论严重后巩膜破裂伤经过及时修复，避免盲目摘除眼球，有望重建视功能。
Objective To investigate treatment of the posterior sclera ruptured with posterior segment severely injured. Methods We retrospectively investigated 19 cases （ 19 eyes） with their posterior segment eye being severely injured from Jan 2004 to Oct, 2005. The length of the wounds were over 10mm. With the intraocular contents lost. All the patients had undergone primary or secondary vitrectomy and silicone oil tamponade after the management of the wound. The following up period was form 3 month to 2 years. Results The pathologic changes of the injured eyes including intraocular hemorrhage, subretinal bleeding, superchoroidal space hemorrhage, retinal detachment, intraocular contents lost, retinal vessel occulsion, optic nerve injury, iris absent and dislocation of the lens. The visual acuity was light perception in 8 eyes and no light perception in 11 eyes before the surgery. The best postoperative visual acuity were no light perception in 4 eyes, light perception in 2 eyes, 2eyes 〈 0.01,0.01≤5 eyes 〈 0.05,0.05≤ 3 eyes 〈 0.3,3 eyes 90.3. Conclusion After being managed by vitreoretinal surgery, most of eyes with severe posterior sclera rupture injury could restore a better visual acuity.
