摘要
目的:分析Rey复杂图形测验与西方失语症成套测验的相关性,探讨Rey复杂图形测验在汉语失语症评估中的应用价值。方法:选择2002-03/2005-09在解放军总医院康复医学科接受住院或门诊治疗的脑损害后失语症患者31例,男28例,女3例。均接受Rey复杂图形测验检查及西方失语症成套测验检查。Rey的图形由重复的正方形、长方形、三角形和各种的其他的形状组成,给予这复杂的结合图形测验患者,试验分数反映了应试者视觉组织和运动计划技能。西方失语症成套测验包括言语性和非言语性功能测验两部分。言语部分的项目有:自发言语、听理解、复述及命名;非言语部分的项目有:阅读、书写、运用和结构。根据检查结果,计算出失语商、操作商和大脑皮质商。对Rey复杂图形测验的临摹分数、临摹时间、回忆分数和回忆时间与西方失语症成套测验中的各分项分值及失语商、操作商、大脑皮质商之间的关系分析采用Pearson相关分析、偏相关分析和逐步线性回归分析。结果:31例脑损害后失语症患者全部进入结果分析。①Rey复杂图形测验的临摹时间与西方失语症成套测验中各亚项及失语商、操作商、大脑皮质商均有显著负相关关系(r=-0.6722~-0.4295,P<0.05),经逐步回归分析,入选为影响临摹时间的主要因素是复述。②回忆分数与西方失语症成套测验中运用、结构呈显著性正相关性(r=0.4333~0.4786,P<0.05),而与西方失语症成套测验中其他各项指标无显著性相关(r=0.0978~0.3998,P>0.05)。③临摹分数、回忆时间与西方失语症成套测验中各项指标无显著相关关系(r=-0.2495~0.3193,P>0.05)。结论:Rey复杂图形测验可应用于脑损害后汉语失语症患者的测验,其中临摹时间对于汉语失语症患者是一项重要观测指标。
AIM: To analyze the correlation between Rey complex figure test and western aphasia battery, and investigate the applying value of Rey complex figure test in evaluate Chinese aphasia.
METHODS: Thirty inpatients or outpatieuts (28 male and 3 females) with aphasia following brain injury were selected from the Department of Rehabilitation Medicine, General Hospital of Chinese PLA form March 2002 to September 2005. They all received Rey complex figure test and western aphasia battery, The Rey figures were composed of overlapping squares, rectangles, triangles, and various other shapes, and this complex combination was given to the patients, the scores of this test reflected the examinees' visual organization and motor planning skills, The western aphasia battery included two parts of language function test (spontaneous speech, listening comprehension, retelling and nomination) and non-language function test (reading, writing, exereise and structure), According to the results of the western aphasia bat tery, the aphasia quotient, performance quotient and cortical quotient were calculated, The as sociations between copy score, copy time, recall scores, recall time in Rey complex figure test and every subentry scores, aphasia quotient, performance quotient.and cortical quotient in western aphasia battery were analyzed with the Pearson correlations analysis, partial correla tions analysis and stepwise linear regression analysis.
RESULTS: All the 31 patients with aphasia following brain injury were involved in the analy Sis of results, (1) The copy time in Rey complex figure test was significantly and positively correlated with every subentry seores, the aphasia quotient, performance quotient and cortical quotient in western aphasia battery (r=-0.672 2 to -0.429 5, P 〈 0.05), and the stepwise linear regression analysis showed that the main factor involved in the eopy time was retelling, (2) The recall scores in Rey complex figure test had significant positive correlation with exercise and structure in western aphasia battery (r=0.433 3 to 0.478 6, P 〈 0.05), but had no significant correlations with the other items in western aphasia battery (r=0.097 8 to 0.399 8, P 〉 0.05). (3) The copy scores and recall time in Rey complex figure test had insignificant correlation with all the items in western aphasia battery (r=-0.249 5 to 0.319 3, P 〉 0.05).
CONCLUSION: The Rey complex figure test may be applied to test the patients with aphasia following brain injury, and the copy time in the Rey complex figure test is a very important observational index for the aphasia sufferer.
