摘要
目的探讨脑卒中昏迷患者血清神经元特异性烯醇酶含量的变化及其与患者昏迷程度和预后的关系。方法以河北医科大学第一医院收治的78例脑卒中昏迷患者和同期31例健康体检者作为观察对象,患者在入院后即刻、对照组在清晨空腹时抽静脉血,以放免法测定血清神经元特异性烯醇酶。治疗15 d后,应用日常生活能力(ADL)中功能独立性评测(FIM)法评估功能恢复程度;应用改良爱丁堡-斯堪的那维亚评分量表评估神经功能缺损。结果正常对照组血清神经元特异性烯醇酶含量为3.5±1.9μg/L,研究组血清神经元特异性烯醇酶含量为28.6±6.5μg/L,两组比较差异有极显著性意义(P<0.001);血清神经元特异性烯醇酶含量与治疗后患者功能恢复程度、神经功能缺损有一定的关系。结论血清神经元特异性烯醇酶含量是判断脑卒中昏迷患者脑损伤程度和评价预后的一个敏感指标。
Objective To explore the changes of serum NSE levels and the relationship between serum NSE levels and coma degree and prognosis in coma patients with stroke, Me.otis levels of NSE of 78 coma cases with stroke in the first hospital of hebei medical university were used as study group, and 31 cases of healthy people were used as control. The vein blood was collected as soon as the patients were admitted and the control people were empty stomach at early morning. The levels of NSE were measured by the method of radioimmunity. 15 days after treatment, the degree of functional recovery was evaluated by Functional independence Measure(FIM) ; Neurologic impairment was evaluated by Improved Edinburgh- Scandinavian Stroke Scale(MESSS). Results Level of NSE of the study group was 28.6±6.5/μg/L and that of control group was 3.5±1.9 /μg/L. Level of NSE of the study group was higher than that of control group (P〈0.001) ; levels of NSE were concered with the degree of functional recovery, neurologic impairment after the patients were treated. Conclutsion S-NSE was a sensitive index to predict prognosis of brain damage.
