急性高容量血液稀释用于全髋置换术的临床研究 被引量:2

摘要 目的:采用6%中分子羟乙基淀粉(6%HES)行急性高容量血液稀释(AHHD),观察其在全髋置换术中节约用血的效力及其对凝血功能的影响。方法:22例病人分为实验组和对照组。记录两组围术期出血量、异体血输注量及补液量,分别测定稀释前、稀释后、术毕、术后第1天、术后第7天的Hb和红细胞压积(Hct)及稀释前、稀释后、术毕凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)。结果:AHHD组稀释后Hb、Hct显著降低,但均在安全范围内,术后第1天和术后第7天Hct、Hb与对照组同期比较差异无显著性。AHHD组稀释后PT、APTT均明显延长,统计学上差异有显著性,但均在正常范围内。两组术中出血量差异无显著性,AHHD组11例患者均完全避免了输注异体血,对照组11例中有10例输血,平均输血量400mL,两组比较差异有显著性。结论:围术期以6%HES行AHHD可显著减少全髋置换术病人异体血输注量,对凝血功能(PT、APTT)无明显影响。
