摘要
目的:将危重烧伤患者常见心理问题的分析和干预作简单的归纳和探讨。资料来源:应用计算机检索中国期刊全文数据库1993-09/2005-03的相关文章,检索词为“烧伤”,二次检索词为“心理”。限定文章为中文。同时检索PubMed1994-01/2005-03的相关文章,检索词为“burninjurypsychotherapy”,同时选择关于烧伤患者心理治疗理论的著作。资料选择:对资料进行初审,选取涉及烧伤患者心理分析和治疗的研究,排除重复研究,对英文文献开始查找全文。资料提炼:共收集到关于烧伤患者心理分析和治疗的文章400多篇,以及相关书籍2部。其中12篇中文文章和4篇英文文章中的相关内容和1部书籍符合标准,进行资料的综合和整理。资料综合:淡漠、恐惧、紧张、焦虑、抑郁、烦躁、寂寞是危重烧伤患者常见的心理特点。创伤后应激障碍是一种延迟或延长的焦虑性反应。建立良好的医患关系和良好的社会支持系统是基本的心理干预措施,常用的心理干预疗法有:认知疗法,放松疗法,支持疗法,生物反馈疗法和自我心理锻炼等,针对烧伤患者不同时期的心态,采取相应的心理干预措施。结论:危重烧伤患者往往处于严重的和程度不一的心理应激状态中,给治疗和康复造成巨大影响。总结烧伤患者常见的心理问题分析,并归纳出多种心理干预方法,强调以预防为主和针对烧伤患者不同时期的心态,适时地做好心理康复的心理干预模式。
OBJECTIVE： To sum up and investigate the frequent psychoanalysis and mental intervention of the severe burn patients. DATA SOURCES： By using computer, the articles about the psychoanalysis and therapy in patients with burn injury published between September 1993 and March 2005 were searched in the China journal fulltext databases （OJFD） with the keywords＂burn injury＂and＂psychology＂in Chinese, Pubmed database was also searched to identify the relevant articles published in English from January 1994 to March 2005 with the keyword of ＂burn injury, psychotherapy＂. Meanwhile, the bookmakings of the theory about psychotherapy of burn injury patients were selected, STUDY SELECTION： Having been primarily looked through, the studies and reviews related to the contents of theory and means of psychoanalysis and therapy in burn injury patients were screened. Those repetitive studies were excluded, and the full-texts of the literatures in English were looked up, DATA EXTRACTION： More than 400 articles and 2 literatures relevant to psychoanalysis and therapy in born injury patients were collected. The 12 articles in Chinese and 4 articles in English and 1 literature were in accordance with the inclusive criteria. Then, the selected data were synthesized and arranged, DATA SYNTHESIS： Indifference, fear, intension, anxiety, depression, restlessness, lonesome are the frequent mental characteristics, Posttraumatic stress disorder is a delayed and prolonged anxious reaction, To establish favourable doctor-patient relationship and favourable social support system elementary mental intervention study, The frequently used mental intervention treatments were： cognitive therapy, relaxation therapy, maintenance therapy, biofeedback therapy, self mental training, and so on, Corresponded mental intervention ＂treatments were adopted to aim directly at the different period of mind in burn patients, CONCLUSION： Severe burn patients are always in the state of severe and miscellaneous psychological stress, It impacts their therapies and restorations, This article summarizes the psychoanalysis of the frequent psychological matters, and sums up many methods of the mental intervention, It emphasizes the precaution and the mode of mental intervention that carrying out psychological restoration to aim directly at the different psychological state of the severe burn patients.
