摘要
目的探讨超声心动图及组织多普勒技术评价自体骨髓单个核细胞(MBMCs)移植对心肌梗死后患者心脏功能的影响。方法选择6例冠心病心肌梗死患者进行自体MBMCs移植,分别于术前以及细胞移植术后6个月内进行心脏超声和99mTc-甲氧基异丁基异腈(99mTc-MIBI)心肌灌注显像检查。根据超声心动图及组织多普勒技术测定心肌梗死区室壁厚度(IWT)、运动速度(IWMV)、左室舒张末内径和收缩末内径、左室射血分数和左室缩短分数、缺血性二尖瓣反流。99mTc-MIBI心肌灌注显像检查后对所得即刻图像和4h延迟图像进行评分。结果自体MBMCs移植术后与术前相比局部心脏收缩功能明显改善,表现为IWT增加,IWMV增加(P<0.05)。自体MBMCs移植术后与术前相比整体收缩功能亦有改善,表现为左室舒张末内径及收缩末内径减小(P<0.01),左室射血分数及缩短分数提高(P<0.05)。自体MBMCs移植术后缺血性二尖瓣反流明显好转(P<0.05)。99mTc-MIBI心肌灌注显像图像评分结果显示,自体MBMCs移植术后与术前相比缺血区心肌灌注明显增强(P<0.05)。结论自体MBMCs植入心肌梗死区及其边缘区可改善心肌梗死后局部和整体收缩功能。
Objective To evaluate cardiac function after transplanting autologus monouclear bone marrow cells (MBMCs) in patients with myocardial infarction by echocardiography and Doppler tissue imaging. Methods Six male patients with myocardial infarction who need coronary artery bypass procedure were prospectively studied. Before cell transplantation and in the follow-up, echocardiography and Doppler tissue imaging was performed to observe the regional and global cardiac function of the patients, including infarction wall thickness (IWT), infarction wall movement velocity ( IWMV), left ventricular end-diastolic diameter (LVDd), left ventricular end-systolic diameter (LVDs), left ventricular ejection fraction ( LVEF), left ventricular shorting fraction(LVFS), ischemic mitral regurgitation. 99m Tc MIBI SPECT examination included immediate and prolonged image scors. Results After transplanting autologus MBMCs, IWT was increased compared with that before cell transplantation( P〈0.05); the regional IWMV was also improved ( P 〈0.05) ;additionally, LVDd and LVDs were smaller than those before cell transplantation( P〈0.01 ) ; LVEF and LVFS were also greater( P 〈0. 05) sand the average ratio of ischemic mitral regurgitation was also alleviated than that before cell transplantation( P〈0.05). Myocardial perfusion of ischemic regions assessed with ^99mTc-MIBI SPECT was also improved ( P 〈0.05). Conclusions The regional and global cardiac function was improved in the patients with myocardial infarction after transplanting autologus MBMCs.
