摘要
目的探讨按摩下腹部、热敷下腹部、听流水声及冲洗会阴等4种方法的诱导排尿效果,以减少或避免因肾穿术后尿潴留而需的导尿。方法选择200例肾穿后发生非梗阻性尿潴留的患者,随机分成4组,分别采取按摩下腹部、热敷下腹部、听流水声、冲洗会阴等方法对患者进行诱导排尿,观察方法实施后患者排尿的时间、诱导排尿的成功率、更换诱导排尿方法的转归情况。结果热敷组平均诱导排尿时间为23min,排尿成功率为40%(20/50);按摩组排尿时间31min,排尿成功率为46%(23/49),听流水声组排尿时间为18min,排尿成功率为60%(31/51);会阴冲洗组排尿时间为10min,排尿成功率为90%(45/50)。对使用前3种诱导排尿法无效的76例患者再使用冲洗会阴的方法,排尿成功率为93%(70/76),组间差异有显著性。结论应用冲洗会阴法对肾穿术后发生的非梗阻性尿潴留进行诱导排尿,效果明显优于其他方法,建议作为临床首选。
Objective To investigate the effect of induced urination of massaging lower abdomen, applying heat on lower abdomen,listening to sound of water stream , perineal area flushing, to avoid retention catheter of patients with retention of urine after nephrocentesis. Methods Two hundred patients were randomly divided into 4 groups with retention of urine after nephrocentesis. Patients in group 1 were given lower abdomen massage, Patients in group 2 were applied heat on Lower abdomen, Patients in group 3 listened to sound of water stream, Patients in group 4 were given perineal area flushing. The time of induced urination, successive rate of induced urination, alternative methods of induced urination were observed respectively. Results The average time of induced urination and the successive rate, in group 1 were 23 minutes and 40%, in group 2 were 31 minutes and 46%, in group 3 were 18 minutes and 60%, in group 4 were 10. 12 minutes and 90%. The patients without effects in group 1,group 2, group 3 were treated with perineal area flushing and the successive rate was 93%. The difference was significant. Conclusions Perineal area flushing has more significantly effect for the patients with retention after nephrocentesis than other methods.
