摘要
目的探讨新型微波刀切除肝癌的可行性。方法腹腔镜下应用HWB-2D型微波刀对20例周边型肝癌行肿瘤切除术。结果20例腹腔镜肝癌切除术均顺利完成,手术时间1.5~4h,中位时间110min。术中出血量20~200ml,平均110ml。2例分别于术后第9、13个月肝癌复发;19例随访,1年生存率为94.7%(18/19)。结论腹腔镜下应用HWB-20型微波刀行肝癌切除术,可有效地控制肝切面的出血;微波刀切割的同时可杀灭可能残留的肝癌细胞,降低了术后肝癌的复发。微波刀肝癌切除术适宜于肿瘤位于肝脏周边(II、III、IVa、V、VI段),直径<10cm,无周围脏器受累,是一种简便、安全、微创而有效的治疗方法。
Objective To explore the feasibility of a new type of microwave knife in the resection of hepatic carcinoma. Methods A total of 20 patients with peripheral hepatic carcinoma were treated by laparoscopic resection using a new type of microwave knife ( HWB - 2D, China). Results The procedures of laparoscopic resection of hepatic carcinoma were successfully accomplished in all the 20 patients, with an operation time of 1,5 -4 h (median, 110 min) and an intraoperative blood loss of 20 - 200 ml ( mean, 110 ml), Two patients had a recurrence of hepatic carcinoma at 9 and 13 postoperative months respectively. Follow - up observations in 19 patients showed a 1 - year survival rate of 94.7% (18/19), Conclusions Use of the HWB - 2D microwave knife in laparoscopic resection of hepatic carcinoma can effectively decrease the blood oozing from hepatic wound surface, minimize the risk of laparoscopic hepatectomy, and shorten the time of operation, Microwave coagulation can kill remnant cancer cells when performing the resection, which decreases the recurrence rate after operation, This procedure is a simple, safe, little invasive, and feasible option for hepatic carcinomas located at peripheral segments (Ⅱ, Ⅲ, Ⅳa, Ⅴ, or Ⅵ) , less than 10 cm in diameter, and without involvement to surrounding organs.
