多普勒组织成像技术对冠心病左室整体功能的研究

摘要 目的应用多普勒组织成像(DTI)技术测量二尖瓣环运动速度,评价冠心病患者的整体心功能.方法对52例确诊冠心病的患者(冠心病组)和36例正常人(对照组)应用DTI从心尖四腔、心尖二腔和心尖长轴切面测定后间隔、侧壁、前壁、下壁、前间隔和后壁6个部位的二尖瓣环收缩期(Sa)、舒张早期(Ea)和舒张晚期(Aa)运动速度峰值,并将两组进行显著性检验.结果冠心病组二尖瓣环后间隔、侧壁、前壁、下壁、前间隔和后壁6个节段Sa(4.82±1.58、5.31±1.79、4.42±1.60、5.00±1.48、4.35±1.77、4.96±1.34) cm/s,低于正常组(5.53±1.35、6.09±1.77、5.57±1.42、6.12±1.24、5.52±1.41、5.85±1.36) cm/s(P<0.05~0.01), Ea(4.24±1.44、5.52±1.99、4.33±1.59、4.32±1.49、4.19±1.65、5.01±1.69) cm/s 和Ea/Aa之比(0.76±0.22、0.95±0.29、0.71±0.31、0.69±0.39、0.67±0.40、0.77±0.34) cm/s低于正常组(5.85±1.65、6.96±2.09、6.26±1.59、6.32±1.82、5.71±2.01、6.43±2.01)、(1.09±0.27、1.16±0.31、1.03±0.36、1.22±0.21、1.25±0.23、1.04±0.29)(P<0.01),而二尖瓣环各节段Aa(5.36±2.29、6.49±1.82、6.12±2.36、7.07±1.90、6.27±1.93、6.75±2.42) cm/s与正常组(5.54±1.59、6.85±1.53、6.37±2.36、7.15±1.75、6.62±2.06、6.53±1.82) cm/s相比差异无统计学意义(P>0.05).结论应用DTI技术测定二尖瓣环运动速度可反映冠心病左室整体功能,具有临床应用价值. Objective To evaluate the global left ventricular systolic and diastolic function of patients with coronary artery disease (CAD) using mitral annular motion velocities measured by Doppler tissue imaging (DTI). Methods Fifty-two patients with CAD (all of them have been confirmed by coronary angiography) and thirty-six normal subjects were studied. The peak systolic velocity(Sa), peak early diastolic velocity(Ea) and late diastolic velocity(Aa) were measured at six mitral annular sites: septal and lateral from apical 4-chamber views, anterior and inferior from 2-chamber views, anteroseptum and posterior from 3-chamber views. Results Compared with the normal subjects, the CAD patients had significantly reduced Sa (4.82 ± 1.58,5.31 ± 1.79,4.42 ± 1.60,5.00 ± 1.48,4.35 ± 1.77,4.96±1.34) cm/s,Ea (4.24±1.44,5.52±1.99,4.33±1.59,4.32±1.49,4.19±1.65,5.01±1.69) cm/s and Ea/Aa ratio(0.76±0.22,0.95±0.29,0.71±0.31,0.69±0.39,0.67±0.40,0.77±0.34) cm/s for each of six mitral annulus sites(P〈0.05-0.01). Aa (5.36±2.29,6.49±1.82,6.12±2.36,7.07±1.90,6.27±1.93,6.75±2.42) cm/s was not significantly different between the two groups( P ±0.05). Conclusion Mitral annular motion velocities measured with DTI should be used to evaluate global left ventricular function.
