摘要
目的:探讨妊娠合并心脏病患者不同心功能状态和心电图改变对母亲和围产儿的影响.方法:对1990年1月～2004年12月间92例妊娠合并心脏病患者的临床资料进行回顾性分析.结果:妊娠合并心脏病发生率为0.74%,孕产妇病死率为0,围产儿死亡率为0.48‰,剖宫产率70.0%,心律失常发生率为76.09%.妊娠合并心脏病患者以心肌炎、心肌炎后遗症的发生率最高(35例,38.04%),其次为先天性心脏病(19例,20.65%).心功能Ⅰ～Ⅱ级与Ⅲ～Ⅳ级比较,妊娠孕周、新生儿平均体重、围产儿患病率及死亡率差异显著(P＜0.01或P＜0.05).结论:妊娠合并心肌炎、心肌炎后遗症的发生率有升高趋势,而风湿性心脏病的发生率已较80年代下降;妊娠合并心脏病易导致心功能不全、心律失常、心肌劳损;同时也影响孕周、新生儿体重,增加剖宫产机率.
Objective： To investigate the influence of cardiac function and electrocardiography of pregnant women on outcome of their pregnancies and themselves. Methods：Clinical data of 92 cases of pregnant women with heart diseases from January, 1.990 to December, 2004 were analyzed retrospectively. Results：The incidence of cardiac disease of heart function was 0.74%.The death rate of pregnant women was 0. The perinatal mortality of mothers was 0.48‰.The incidence of cesarean section was 70% .The abnormal cardiac rhythm was 76.09%.The highest incidence of cardiac disease of heart function was due to myocarditis, sequelae of myocarditis（35 cases,38.04% ）.The incidence of congenital heart disease is on the second （19cases,20.65%）.Cardiac disease with Grade I-Ⅱ of heart function was quite different from that of Grade Ⅲ-Ⅳ in pregnant length,average weight of new bores, the incidence of sick and perinatal mortality outcome of the pregnancies. （P 〈 0.01 or P 〈 0.05）. Conclusion： The incidence of myocarditis and sequelae of myocarditis is increasing while the rheumatic heart diseases is dropping compared with that in the 1960s. Cardiac disease can easily cause the cardiac dysfunction and abnormal cardiac rhythm, mycocarditital strain, and it can also effect the length of pregnant,the weight of new borns,inereasing the incidence of cesarean section.
