摘要
目的:观察带锁髓内钉治疗肱骨骨折的疗效,分析并发症发生的原因并提出防治措施.方法:运用扩髓带锁髓内钉治疗肱骨骨折67例,随访6个月～3年,平均13个月.结果:64例骨折愈合(其中4例迟缓愈合),骨折愈合时间3～7月,平均愈合时间3.8月,根据术后X线片观察骨折愈合情况及肩肘关节功能综合判定疗效,优32例,良26例,差9例.并发症16例:感染1例,锁钉退出2例,骨折不愈合3例,医源性粉碎性骨折3例,肩痛症或肩、肘关节功能受限7例.结论:带锁髓内钉治疗肱骨骨折疗效可靠,在正确使用下,熟练操作,能减少并发症的出现,具有手术创伤小,固定可靠,术后早期功能煅练等优点.
Objective： To observe the clinical effect of the treatment for the humeral fractures with interlocking nails, and to analyze the causes of the complication and explore measures of the prevention. Methods： 67 cases of humeral fractures were treated with interlocking nail, following time is 6months to 3 years, average 13months. Results：64 cases of fractures were healed（4 eases delayed healing）. average healing time is 3months to 7months, average healing time 3.8months. refer to han＇ s method of observation for fracture healing by X- ray postoperation and shoulder and elbow joint function to judge clinical effect, superior 32cases, fine 26cases, inferior 9cases. 16 cases come with complication：infection leases, withdraw of locking nails 2cases, unhealing of fracture 3 cases, medical comfined fracture fracture 3cases, shoulder arthritis or dysfunction of shoulder and elbow joint 7cases. Conclusion： Interlocking nail is an excellent alternative for the treatment of femoral shaft fractures, correct and skilled use of interlocking nail can reduce complication, it has the superior of little wound, reliable fixation , early exercise postoperation etc.
