参芪扶正注射液配合化疗治疗急性白血病疗效及对T淋巴细胞亚群和血清IFN-γ、IL-10、IL-2水平的影响 被引量:10

摘要 目的观察参芪扶正注射液配合化疗治疗急性白血病患者疗效及对T淋巴细胞亚群(CD4 、CD8、CD4 /CD8)和血清干扰素-γ(IFN -γ)、白细胞介素(IL) - 10、IL -2的影响。方法将6 5例初治白血病患者随机分为参芪扶正注射液加化疗组(参芪组,32例)及单纯化疗组(对照组,33例) ,观察其缓解率、治疗前后外周血成熟中性粒细胞绝对值的变化、T淋巴细胞亚群和血清IFN- γ、IL -10、IL- 2水平。结果治疗后参芪组缓解率与对照组比较,差异无显著性(P >0. 0 5 )。化疗后第1、2、3周末两组外周血成熟中性粒细胞绝对值均降低,与本组化疗前比较,差异均有显著性(P <0 . 0 1或P <0 . 0 5 ) ;第3、4周末两组成熟中性粒细胞绝对值较化疗后第1、2周回升,且参芪组高于对照组,两组比较差异有显著性(P <0 . 0 5 )。治疗后两组CD4 、CD4 /CD8均明显提高,与治疗前比较,差异有显著性(P <0 .0 5 ,P <0 . 0 1)。治疗后两组血清IFN -γ、IL- 2水平明显升高,血清IL -10水平明显下降,与治疗前比较,差异均有显著性(P <0 . 0 1) ;两组治疗后比较,差异亦有显著性(P <0 . 0 5 )。结论参芪扶正注射液能改善、调节接受化疗的急性白血病患者的免疫功能,促进化疗后骨髓造血细胞增生,增强疗效。 ObjectiveTo investigate the clinical efficacy of Shenqi Fuzheng Injection (SFI) combined with chemotherapy in treatment of patients with acute leukemia and its effect on the levels of T-lymphocyte subsets (CD_4,CD_8,CD_4/CD_8) and serum interferon -γ(IFN-γ), interleukin-10 (IL-10) and IL-2. MethodsSixty-five patients with initial treating acute leukemia were randomly divided into 2 groups, the SFI group (n=32) treated with SFI plus chemotherapy (CT), the control group (n=33) treated with CT only. The remission rate, changes of peripheral mature neutrophilic granulocyte (PMNG) count , T-lymphocyte subsets, serum IL-10 and IL-2 before and after treatment were determined. ResultsThe remission rate in the two groups showed no obvious difference (P>0.05). After CT for the 1st, 2nd and 3rd weeks, the PMNG count decreased in both groups, showing significant difference as compared with that before CT (P<0.01 or P<0.05). The PMNG count at the end of the 3rd and 4th week of CT remounted to higher than that at 1st and 2nd week, and the increment in the SFI group was significantly higher than that in the control group (P<0.05). The levels of CD_4, CD_4/CD_8, IFN-γ and IL-2 all increased in the two groups after treatment (P<0.05,P<0.01), however, that of IL-10 was significantly decreased (P<0.01). The difference between the two groups in these criteria after treatment was also significant (P<0.05). ConclusionSFI can improve and regulate the immune function of the patients with acute leukemia undergoing CT, it could promote bone marrow cells proliferation and enhance the efficacy.
