摘要
目的了解广州地区1999~2003年鲍曼不动杆菌耐药性及变化趋势,以指导临床合理用药。方法严格按照NCCLS的标准,用K-B法对从广州地区13家医院收集的鲍曼不动杆菌进行药物敏感性实验。用WHONET5软件进行资料统计,SPSS统计软件卡方检验进行统计学统计。结果1999~2003年不动杆菌对12种受试抗生素的耐药率分别是亚胺培南(IMP)7.3%~10.7%,阿米卡星(AMk)21.8%~34.1%,头孢匹美(FEP)21.4%~42.0%,头孢他啶(CAZ)26.2%~40.9%,左氧氟沙星(LEV)21.2%~43.0%,环丙沙星(CIP)36.0%~47.1%,庆大霉素(GEN)42.3%~48.9%,复方新诺明(SXT)39.0%~57.1%,哌拉西林(PIP)55.3%~67.0%,四环素(TEY)66.0%~81.0%,头孢噻肟(CTX)88.3%~90.8%及氨曲南(ATM)89.3%~97.1%。鲍曼不动杆菌对AMK、LEV、FEP、CIP、SXT、TEY、ATM的耐药性在1999~2003年间有差异(P<0.05),而对IMP、CAZ、GEN、PIP、CTX的耐药性无显著差异即无多大的变化(P>0.05)。结论IMP是广州地区治疗不动杆菌感染的最有效的抗生素。临床常用的各抗生素之间存在严重的交叉耐药,部分抗生素耐药性有上升趋势,临床应加强合理用药。
Objective To investigate the trend of antibiotic resistance of Acinetobacter baumannii(Abi )in Guangzhou during 1999~2003and to di rect clinical treatment.Methods Antimicrobial susceptibility test(AST)of Abi was done by K-B.The results were read according to the standard of NCCLS.T he data were processed by WHONET 5and the statistics were done by SPSS.Results During 1999~2003the rates of resistance of Abi to 12antibiotics respectively were Imipenem(IMP )7.3%~10.7%,Amikacin(AKM)21.8%~34.1%,Levofloxacin 21.2%~43.0%,Cefepime (FEP)21.4%~42.0%,Ceftazidime (CAZ)26.2%~40.9%,Ciproflozacin(CIP)36.0%~47.1%,Gentamycin(GEN)42.3%~48.9%,Sulfamethoxazole-Tr imethoprin(SXT)39.0%~57.1%,Piperacillin(PIP)55.3%~67.0%,Tetracycline(TET)66.0%~81.0%,Cefotaxiame (CTX)88.3%~90.8%,and Aztreonam(ATZ)89.3%~97.1%.Acinetobacter baumannii showed significant difference in th e resistance to AMK,LEV,FEP,CIP,SX T,TEY and ATM during1999~2003(P<0.05),but no significant difference to IM P,CAZ,GEN,PLP and CTX(P>0.05).Conclusions IMP is the valid classical antibiotics to Acinetobacter baumannii.There are relatively serious cross resistance among the antibiotics which are commonly used in clinical treatment in Guangzhou.There is a trend of increase in the resistance to some antibiotics.The clinical doctors should carefully u se antibiotics for treating the dise ase.
