氟西汀对脑卒中后抑郁状态及认知功能的影响 被引量:52

摘要 目的 探讨氟西汀对脑卒中后抑郁状态及其认知功能的影响。方法 将82例脑卒中后抑郁患者随机分为研究组41 例,对照组41例。研究组在神经内科常规治疗的基础上,入组前停服其他抗抑郁剂,并进行1w清洗期后服用氟西汀,未服用 抗抑郁剂者直接服用氟西汀,给药剂量为20~40mg·d-1;对照组进行神经内科常规治疗。两组均在治疗第1w内、第6w末 采用抑郁自评量表和P300电位进行测定。结果 治疗第1w内两组抑郁自评量表分值及P300电位测定无显著性差异(P> 0.05),治疗第6w末研究组抑郁自评量表分值显著低于对照组(P<0.01);研究组P300电位成分中N2、P3波潜伏期缩短和 P波幅升高较对照组明显(P<0.05)。结论 氟西汀是一种较理想的抗抑郁药物,对改善卒中后抑郁及认知功能效果明显。 Objective To explore the effects of fluoxetine on post-stroke depression and its cognitive function. Methods 82 patients with post-stroke depression were randomly divided into two groups: research group(n=41,routine treatment combined with fluoxetine) and control group(n=41, routine treatment only),the Self-rating Depression Scale(SDS) and P300 potentials test were used for the two groups in 1 st week and the end of 6 th week of treatment. Results The SDS score and P300 potentials had no significant difference between the two groups in the 1 st week (P>0.05),and at the end od 6 th week the score of SDS in research group was significantly lower than that in control group(P<0.01).The latency of N 2 and P 3 of P300 potentials were shorter (P<0.05)and the amplitude of P 3 of P300 potentials higher (P<0.05) in research group than those in control group. Conclusion Fluoxetine is an ideal antidepressant improving post-stroke depression and cognitive function.
