经尿道等离子体双极电切术治疗良性前列腺增生 被引量:17

Transurethral resction of hyperplastis prostate using bipolar plasmakinetic technique

摘要 目的评价经尿道等离子体双极电切术治疗良性前列腺增生(BPH)的疗效.方法用经尿道等离子体双极电切行前列腺切除(PKRP)200例,病人年龄58~91岁,平均74.1岁,前列腺32~146g,平均(56.3±40.2)g.术后随访1~6个月.结果手术时间16~103min,平均(52±32)min,无一例需要输血,无电切综合征发生.术后IPSS由(27.1±4.5)分降至(11.3±3.4)分(P<0.05);最大尿流率(MFR)由(6.1±2.4)ml/s上升至(1 8.6±3.5)ml/s(P<0.05);生活质量评分(QOL)由术前的5.6下降至1.5(P<0.05);剩余尿(RU)由(102.3±43.3)ml降至(22.6±16.3)ml(P<0.05).4项指标手术前后比较差异均有显著性意义(P<0.05).结论用双极等离子体进行PKRP安全有效. Objective To evaluate the validaty of bipolar plasmakinetic technique (PKRP) for the treatment ofBPH. Methode 200 patients(mean age of 74.1 years;ranging from 58 to 91) with symptomatic BPH without suspectedcancer (confirmed by IPSS ,urinary flow rate, ultrasonographic estimate and PSA tests) were treated by PKRP. Result ForPKRP, the duration of the procedure was (5232)min (range 16~103 min). The patients have been followed up for 1~6 mouthspostoperatively. No cases need blood transfusion during operation. No transurethral resection syndrome occurred. Themaximun flow rate (MFR) was elevated from (6.12.4)ml/s to (18.63.5)ml/s(P<0.05), IPSS, RU and QOL beingdropped from 27.14.5 to 11.33.4, (102.343.3)ml to (22.616.3)ml(P<0.05) and from 5.6 to 1.5 (P<0.05)respectively. Conclusion Transurethral bipolar plasmakinetic resection of the prostate is effective and safe.
