摘要
目的探讨小儿肠系膜囊肿临床表现及诊治要点.方法回顾性分析1993～2003年我院经手术及病理证实17例小儿肠系膜囊肿临床表现、诊断及治疗资料.结果男女比例大致相等.发病年龄3岁以内35.3%,6岁以内70.6%,9岁以内94.1%.腹痛、腹胀、腹块是本病三大临床特点.全组入院诊断符合率35.4%,误诊为大网膜囊肿(23.5 %)及化脓性阑尾炎或全腹膜炎(17.6 %).CT检查35.3%,诊断符合率100%;腹部X线平片检查41.3%,诊断符合率42.9 %;B超检查94.1%,诊断符合率93.8 %.囊肿位于空肠35.3%,回肠29.4%,结肠35.3%.行囊肿及紧邻肠系膜、肠管切除术82.3%,囊肿完整摘除术11.8%,囊肿大部分切除、残余与血管黏连处囊肿内表面用碘酊烧灼5.9%.结论 10岁以内儿童有腹痛、腹胀、腹块表现者应警惕本病可能;B超和CT检查诊断价值高;手术治疗作为首选,近期随访疗效满意,3年随访7例(41.2%)无复发.
Objective To study the clinical,diagnosistic and treatmental features of mesentric cysts.Methods Clinical,diagnostic and treatmental data of 17 patients of mesenteric cysts in children were reviewed retrospectively. Results Mobidity between male and female is equal.Rate of attacking age is 35.3% under 3 years,70.6% under 6 years and 94.1% under 9 years .Pain,distension,and mass of abdomen are the main clinical situations.The diagnostic accuracy rates is 35.4%,and the misdiagnosis rates to greater omentum cysts and appendicitis (or peritonitis) are 23.5% and 17.6%respectively at admission.35.3% patients were checked by CT,the accuracy rate is 100%.41.3% patients were checked by plain film of abdomen,the accuracy fate is 42.9%. 94.1 % patients were checked by ultrasound ,the accuracy rate is 93.8%.35.3%,29.4% and 35.3% parients′ cysts were located in jejunum mesentery,ileum mesentery and colon mesentery respectively.82.3% patients were conducted with bowel,mesentery and cysts resection.11.8% patients were conducted with cyst resection singly .5.9% patients were conducted with half-baked cyst resection,and the surface on inta-cyst and vessel adhesion was treated with tinctura iodi.Conclusion Children under 10 years who have the presenting features of abdominal pain,distension and mass should be suspected of mesentric cysts.Ultrasound and CT are the most valuable methods for diagnosis.Operation is the first choice of cure,and the effect of therapy is satisfied.Seven cases have not relapsed during 3 years.
