摘要
目的探讨强直性脊柱炎后凸畸形全脊椎楔形截骨矫正术后并发应激性溃疡的相关因素及预防措施。方法回顾性分析5例强直性脊柱炎后凸畸形矫正术后并发应激性溃疡患者的临床资料。结果本组5例患者,经去除应激因素、胃肠减压、全身支持等综合治疗后,均得到了临床治愈。无死亡病例发生。结论强直性脊柱炎后凸畸形矫正术后并发应激性溃疡是一种严重并发症。对长期服用非甾体类消炎药物患者,围手术期应预防性使用胃粘膜保护剂及抑制胃酸分泌药物,防止应激性溃疡的发生。
Objective: To investigate the intrinsic factors and prophylaxis methods of stress ulcer after Wedge-shaped osteotomy surgery correction of total spine of ankylosing spondylitis kyphosis deformity. Method:The clinical feature of five patients eliminated stress ulcer after correction of ankylosing spondylitis kyphosis deformity were retrospectively reviewed.Result: The five patients recovered after eliminating stress factors,drainage with nasogastric tube and general supporting treatment.There wre no dead cases patients.Conclusion:Stress ulcer after surgical correction of ankylosing spondylitis kyphosis deformity is a severe postoperative complication. For the patients who had taken in non-steroidal anti-inflammatory drugs(NSAIDs) for a long time,protective agent of gastric mucosa and restrain gastric acid secretion should be taken in the former after operation to avoid stress ulcer.
