摘要
心脏记忆是近20年来才被人们开始认识的一种正常的生理现象。在未出现心力衰竭、心肌缺血等疾病,心电图未见异常的情况下,若给心脏施加易位电刺激一段时间,如心室起搏在恢复为正常窦性节律后心电图T波仍与起搏时QRS波同向及起搏停止后T波记住了起搏时QRS波形态,且这种记忆维持一段时间后才会消失。由于T波的记忆现象会影响心室复极和动作电位的不应期,有诱发或抑制心律失常发生的作用,且由它引起的T波形态的改变,与许多病理状态下,如心肌缺血时T波的形态相似,容易产生混淆等特点,使心脏记忆的研究越来越受到人们的关注。本文重点介绍了心脏记忆现象及其产生的电生理离子通道机制,并总结了当前的主要研究结果。
Cardiac memory is an normal phenomenon which is known only for about twenty years. In the absence of heart diseases such as heart failure and myocardial ischemia, the T wave inversions appear after return of normal depolarization in leads where the T wave were normal before pacing. The T wave remember the QRS wave of pacing and it will disappear after lasting a period of time. Because it has impacts on the ventricular repolarization and refractoriness, potentially on the expression (or prevention) of arrhythmias, and the changes of the T wave induced by it are easily confused with many pathologic changes such as ischemia .The study on cardiac memory is becoming more and more important. This paper introduced the cardiac memory and its mechanisms at the level of the action potential and ion channels, and sumarized the update major research in this field.
