摘要
目的 探讨在 β 内酰胺类抗生素 /舒巴坦联合制剂对鲍曼不动杆菌的抑菌作用中 ,舒巴坦发挥何种作用。 方法 将舒巴坦配制成 10 μg/片和 30 μg/片 2种浓度的纸片 ,应用K B法检测舒巴坦单剂纸片以及氨苄西林 /舒巴坦、头孢哌酮 /舒巴坦对曼不动杆菌的抑菌圈直径 ,同时增加了舒巴坦分别与哌拉西林、替卡西林、头孢唑林、头孢呋辛、头孢三嗪、头孢噻肟等 6种药物组成的复合纸片。并选择菌株进行舒巴坦对鲍曼不动杆菌的最低抑菌浓度试验 ,用革兰染色法和电镜观察鲍曼不动杆菌的菌体及细胞壁的改变。结果 无论是氨苄西林 /舒巴坦、头孢哌酮 /舒巴坦还是另外 6种 β 内酰胺类药物与舒巴坦的组合制剂的抑菌圈直径与舒巴坦单剂的抑菌圈直径相比均无显著性差异 (P >0 .0 5 )。革兰染色光镜下可见菌体改变为长丝状 ,电镜下可见在亚抑菌浓度时细胞壁缺失。结论 对鲍曼不动杆菌体外的抗菌试验显示 ,舒巴坦对该菌的细胞壁损伤在复合制剂中起主要作用 ,和舒巴坦与何种主药组合对 β 内酰胺酶的抑制作用无明显相关性。
Objective To study the activities of Sulbactam (SB) combined β-lactam antibiotics against Acinetobacter-baumannii in vitro.Method Sulbactam was prepared to 10 μg/disk (SB10) and 30 μg/disk (SB30),and detected by KB method and compared with Ampicillin/SB and Cefoperazone/SB (CFP/SB).The other six antibiotics,including Pipercillin (PRL),Ticarcillin (TIC),Cetazolin (KZ),Cepuroxine (CXM),Ceftriaxone (CRO),Cefotaxime (CTX),combined with SB were also compared in the K-B test.Some isolates were selected to test the minimal inhibitory concentration (MIC) of SB.The changes of the bacterium body and cell wall of the isolates were observed by Gram stain and electron microscope.Results There were no significant differences among SB10,AMP/SB,PRL/SB,and TIC/SB in penicillins group (P>0.05) and SB30,KZ/SB,CXM/SB,CFP/SB,CRO/SB and CTX/SB in Cephems group(P>0.05).Longer cell bodies and destroyed cell walls were observed.Conclusions From the study in vitro,Sulbactam plays a major role on the damage of cell wall against Acinetobacter-baumannii,and there is no obvious correlation for such role of SB as an inhibitor to β-lactamases when SB combined with whatever β-lactam antibiotics.
