颈椎椎体次全切钛网钉板系统的临床应用 被引量:1

摘要 目的 探讨颈椎前路椎体次全切钛网钉板做植骨融合的临床效果。方法 自 2 0 0 2年 3月至 2 0 0 3年 3月间应用颈前路椎体次全切钛网钉板植入融合治疗颈椎管狭窄疾病 1 8例 ,其中 2例患者作了 2节椎体次全切 ,3节椎间隙减压。术后观察减压、固定、融合及神经功能恢复情况 ,并行 X线或 CT检查。结果 患者获 6~ 1 2个月随访 ,神经功能得到不同程度改善 ,无加重情况。椎间隙无高度丢失、无成角 ,均获得骨性融合。 3天后在颈围领固定下下床活动 ,4周后可恢复较轻工作。结论 此术式可避免传统手术方法的缺点 ,不取髂骨 ,融合率高 ,稳定性好 ,并减压彻底 ,疗效好 。 Objective To explore the clinical efficacy of anterior cervical spine subtotal corpectomy with titanium mesh cage combined with screw plate system. Methods 18 patients with cervical spinal stenosis were treated by iliac crest auto graft with titanium mesh cage and screw plate for anterior cervical spine subtotal corpectomy from March 2002 to March 2003, including 2 cases undergone 2 vertebra subtotal corpectomy and 3 intervertebral height decompression through the disc space. Physical examination, X ray and CT were taken at regular interval postoperatively to assess the recovery of nervous function, anterior decompression, cervical stability and bony fusion. Results The patients were followed up for 6~12 months. The nervous functions were recovered in different levels without deterioration; intervertebral height was not decreased and had no angulations. The patients fixed with collar after operations could get out of bed after 3 days and recover work lightly after 4 weeks. All cases acquired bony fusion. Conclusions Titanium mesh cage combined with screw plate system for cervical spine subtotal corpectomy has advantages of less trauma for graft, good stability, higher fusion rate, full decompression, satisfactory clinical efficacy over the traditional ways. Therefore, this operative method is a new method and deserves further application in clinic.
