摘要
目的探讨吻合器痔上粘膜环切术(PPH)治疗混合痔的临床疗效。方法回顾性总结了吻合器痔上粘膜环切术治疗48例临床资料,其中性37例,女性11。结果手术时间平均15分钟,术后平均住院时间3～7天,术后正常生活时间为6天,97%的病人对手术效果满意。有6例术后并发出血(12%),经保守治疗痊愈。尿潴留25例(52%),肛门疼痛16例(16%),下腹部不适及胀痛5例(10%)。随防1～10个月,无肛门铗窄,无大便失禁的发生,效果良好。结论与传统手术相比,PPH安全有效,具有术后疼痛轻,住院时间短和恢复快的优点,且是治疗重度痔近期疗效优越的一种微创手术,但其中远期疗效需进一步观察。
Objeetive To study the clinical effects of procedure for prolapse and haemorrhoids(PPH)in the treatment of Mixed haemorrhoids.Methods Retrospective study of clinical datas of PPHin the treatment of 48 cases of mixed haemorrhoids,male,37 and female,11.Result The operationlasted 15 minutes on average the hospitalization time averaged between 3 to 7 days,and the normal lifewas regained six days after operation.97 percent of patients were satisfied with the operation .6 caseswere had haemorrhage after operation (12 percent),and recovered after comservatine therapy.25patients had urine reteusion(52 percent),16 patients had anal pain(16 percent);After one yearfollow-up,no anal stenosis and feacal incontinence were observed.conclusion Compared with tradi-tional approach ,PPH was safer and more effective ,it showed the advantage of slighter post-operationpains, shorter post-operation hospitalization,and faster recovery,what is more ,for the treatmentof severe haemorrhoids,PPH had a superior short-term cure-rate with trauma,but its longrange effecivenessremained to the study.
