男性冠心病患者ApoE基因多态性与疾病严重程度的关系 被引量:7

Relationship between apolipoprotein E polymorphism and severity of disease in male patients with coronary heart disease

摘要 目的:研究在男性ApoE的基因多态性与冠心病严重程度的关系。方法:采用病例-病例对照的设计方法,对经选择性冠状动脉造影证实为冠心病的108例男性患者应用聚合酶链反应技术确定ApoE的基因型,分析比较ApoE的基因多态性与病情的关系。结果:E2/2,E2/3,E2/4,E3/3,E3/4和E4/4基因型分别占1.85%,10.19%,5.6%,39.81%,37.04%,5.6%,ε2,ε3和ε4等位基因频率分别为9.72%,63.43%和26.85%。E3基因型的HDL水平明显高于E2(P=0.005)。E4基因型患者的病变最长长度明显长于E2基因型者(P=0.023),病变积分E4基因型者明显大于E2,E3者(P均=0.014)。多重回归分析显示病变积分与病变支数、范围及基因型明显相关(b=8.118,10.356,5.187;P=0.002,0.020,0.056),Logistic回归分析显示舒张压、胆固醇浓度、三酰甘油浓度、HDL-C浓度和病变积分与基因型明显相关(b=0.079,0.893,-1.014,-3.413,0.049;P=0.033,0.032,0.027,0.040,0.039)。结论:男性冠心病患者ApoE的基因多态性影响血脂代谢和病情,与疾病严重程度密切相关。 AIM:To study the relationship between polymorphism of apolipoprotein E (AproE) and disease severity in male patients with coronary heart disease (CHD). METHODS:The case case study was used,108 male patients with CHD,diagnosed by selective coronary angiography,were determined Apolipoprotein E genotype by using polymerase chain reaction(PCR),and the relationship of ApoE polymorphism with CHD severity was analyzed and compared. RESULTS:The genotypes of E2/2,E2/3,E2/4,E3/3,E3/4 and E4/4 were 1.85%,10.19%,5.6%,39.81%,37.04%and 5.6%,respectively; the alleles frequencies of ε2,ε3 and ε4 were 9.72%,63.43%and 26.85%,respectively.The high density lipotrotein(HDL) level of E3 genotype was obviously higher than that of E2 genotype(P=0.005).The longest length of lesion in patients with E4 genotype was obviously longer than that of patients with E2 genotype(P=0.023).The integral of lesion in patients with E4 genotype was significantly higher than those in patients with E2 and E3 genotype(P=0.014).Multiple regression analysis showed that the integral of lesion was significantly related with the number and range of lesioned coronary artery and AproE genotype(b=8.118,10.356,5.187;P=0.002, 0.020, 0.056).Logistic regression analysis showed that diastolic pressure, total cholesterol,triglyceride,HDL cholesterol level and integral of lesion were significantly related with ApoE genotype(b=0.079, 0.893,-1.014,-3.413,0.049;P=0.033,0.032,0.027,0.040, 0.039).CONCLUSION:ApoE polymorphism influences the lipoprotein metabolism and severity of disease,and it is significantly related with the severity of CHD in male patients.
