能量多谱勒血流显像诊断肝脏良恶性肿块的价值

摘要 目的 探讨能量多谱勒血流显像 (PDI)对肝脏良恶性肿块的诊断价值。方法 对肝脏 10 2个肿块先用二维超声显示最佳图像后 ,在同一超声切面启用PDI和彩色多谱勒血流显像 (CDFI) ,分别检测肝肿块血管数、血流性质、最大血流速度 (Vmax)及阻力指数 (RI)等。结果 肝脏 10 2个肿块PDI和CDFI血流显示率分别为 74 .5 %(76 / 10 2 )和 4 2 .2 % (6 1/ 10 2 ) ,肝脏良性肿块PDI和CDFI血流显示率分别为 5 9.4 % (38/ 6 4 )和 4 2 .2 % (2 7/ 6 4 ) ,肝脏恶性肿块PDI和CDFI血流显示率分别为 10 0 % (38/ 38)和 89.5 % (34/ 38) ,各组相对应进行比较 ,有显著差异(P <0 .0 0 1)。肝脏良、恶性肿块的RI均值分别为 0 .5 6和 0 .71。结论 PDI对肝脏肿块的血流显示明显优于CD FI。 Objective To investigate the diagnostic value of power doppler imaging(PDI)for benign and malignant neoplasms of the liver.Methods After each best figure of the 102 hepatic neoplasms were shown by 2-dimensional ultrasound,the number of the blood vessel, the character of the blood flow,the velocity of the blood flow and RI in the hepatic neoplasms were detected by PDI and CDFI in the same section.Results The presentation rate of the blood flow in 102 pieces of neoplasms with PDI and CDFI were 74.5%(76/102)and 42.2%(61/102),64 pieces of benign hepatic neoplasms and 38 pieces of malignant hepatic neoplasms shown by PDI were 74.5%(76/102),59.4%(38/64)and 100%(38/38)respectively,but shown by CDFI were 42.2%(61/102),42.2%(27/64)and 89.5%(34/38)respectively,which revealed the significant deviation between PDI and CDFI(P<0.001).The average of RI in the benign and malignant neoplasms of heparic by PDI were 0.56 and 0.71.Conclusion Showing the blood flow of the hepatic neoplasms by PDI were better than by CDFI,and diagnosis of the benign and malignant neoplasms of the liver by PDI have some clinical significance.
