摘要
笔者报道经尿道前列腺电切术中并发症产生原因及护理对策,回顾分析2001年1月～2003年1月162例经尿道前列腺电切术患者的临床资料,有5例患者出现寒战,7例术中出血量超过200ml,3例出现经尿道前列腺电切术综合征先兆,分析其原因:(1)术中冲洗液经手术创面大量、快速吸收;(2)术中灌洗液温度过低使大量体热丧失造成体温降低,出现寒战;(3)出血量多主要与手术时间长及切除前列腺组织多有关.采取相应护理措施:(1)为减少或避免灌洗液的大量吸收采用低压持续灌洗法,密切观察经尿道前列腺电切术综合征的先兆;(2)加强出血的观察与判断;(3)防止病人体温下降.认为护士在术中的密切观察与熟练配合对减少并发症的发生有着重要意义.
The authors reported the causes and nursing measures of the complicated by transurethral resection of the prostate. They retrospectively analyzed the clinical data of 162 patients undergoing transurethral resection of the prostate, finding shivering occurred in 5 cases, blood loss more than 200 ml in 7, and TURS signs in 3. Through analysis, they concluded the causes were as follows: (1) The washing fluid at operation was absorbed through wounds in large amount and at great speed; (2) The lower temperature of douching fluid took away more bodily heat, leading to decrease of body temperature and then shivering; (3) Loss of blood in large amount was associated with the long time and the resection of multiple tissues of the prostate. The nursing measures included:(1) Avoidance of douching fluid'absorption using low pressure continuing douching and close observation of the signs of complications; (2) strengthening the observation and judgment of bleeding; (3) preventing the decrease of their body temperature. They believed the close observation and skilled cooperation at operation is of great significance for the reduction of complications.
