摘要
目的评价腹腔镜肾盂离断成形术并肾脏折叠术治疗肾盂输尿管连接处狭窄(UPJO)所致巨大肾积水的疗效。方法共28例,年龄17～63岁,平均年龄42.7岁,病程1～12年,平均6.4年,男性12例,女性16例,左侧10例,右侧14例,双侧4例。所有病例均经临床及影像学检查获得诊断,并行腹腔镜肾盂离断成形术并肾脏折叠术治疗。结果 28例手术均获成功,手术时间80～160 min,平均120min,术中出血20～80 ml,平均45 ml,无中转开放病例。术后引流管留置3～6 d,平均4.8 d,住院时间6～10 d,平均7.8 d,无尿漏及其它并发症发生。随访6～15个月,B超或CTU提示肾脏积水明显减轻或消失,无吻合口狭窄。结论腹腔镜肾盂离断成形术并肾脏折叠术是治疗UPJO所致巨大肾积水有效可行的手术方法。
Objective To investigate the safety and efficacy of laparoscopic dismembered pyeloplasty and nephroplication in treatment of the giant hydronephrosis caused by ureteropelvic junction obstruction. Methods28 cases with giant hydronephrosis caused by ureteropelvic junction obstruction were all confirmed by physical examination and radiology examination. Laparoscopic dismembered pyeloplasty and nephroplication were performed by one single surgeon from August 2008 to February 2010.Results28 procedures were performed successfully without serious complications and conversion to open surgery.The mean time was 120 min(80 to 160 min) with mean blood loss 45 ml(20 to 80 ml).The postoperative hospital stay averaged 7.8 days(ranging from 6 to 10 days).Follow-up(6 to 15 months) indicated the relief or disappearance of hydronephrosis with no anastomosis stricture.ConclusionsLaparoscopic dismembered pyeloplasty and nephroplication is a safe,effective,feasible and promising method to treat the giant hydronephrosis caused by ureteropelvic junction obstruction.
