摘要 目的 探讨电磁影像导航技术在胸腰椎手术中的应用价值。 方法 选择 32例胸腰椎手术病人 ,其中男 14例 ,女 18例 ;平均年龄 41岁 ;诊断为腰椎滑脱、腰椎管狭窄症、退行性腰椎侧凸等 ,在其手术中均应用VTI电磁影像导航技术辅助进行椎弓根螺钉植入 ,10例同时行椎间融合器植入。术后行植入螺钉节段的X线平片及CT扫描以确定螺钉植入情况。 结果  32例安全完成手术 ,手术时间平均 2 8h(1 6~ 3.7h) ,无与螺钉相关的并发症发生 ,术后影像学检查结果显示所有螺钉植入位置准确。 结论 应用电磁影像导航技术 ,可以确保螺钉的准确植入 ,减少了手术时间及放射线照射 ,尤其对畸形、严重退变及再次手术 ,具有显著的优越性。 Objective To evaluate the efficacy of magnetic imaging guidance for thoracic-lumbar spine surgery. Methods There were 32 patients who underwent thoracic-lumbar spine surgery. There were 14 males and 18 females with an average age of 41 years. The diagnosis of these patients included spondylolisthesis, lumbar spinal stenosis and degenerative lumbar spine scoliosis. All patients underwent pedicle screw insertion utilizing VTI magnetic imaging guidance technique and 10 of these patients had interbody fusion cages. X-ray and CT scan were performed at operated levels post-operatively to evaluate the accuracy of the pedicle screw insertion. Results All patients underwent surgery safely and the average surgery time was 2.8 hours. There was no screw related complications occurred and all screw were inserted accurately. Conclusions The application of magnetic imaging guidance can ensure the accurate placement of pedicle screw in thoracic-lumbar spine surgery and decrease the surgery time and radiation exposure during the surgery. Magnetic imaging guidance has significant advantages especially in the surgery of deformity, severe degenerative disorder and revision of the thoracic-lumbar spine surgery.
