前路病灶清除后路椎弓根固定治疗胸腰椎结核效果观察 被引量:2

Surgical Treatment of Thoracolumbar Spinal Tuberculosis by Anterior Focal Cleaning and Posterior Pedicle Arcus Vertebrae Fixation

摘要 目的总结前路病灶清除后路椎弓根固定治疗胸腰椎结核的临床效果。方法2005年3月—2007年8月共手术治疗胸腰椎结核31例,均采用前路病灶清除后路椎弓根固定术。根据术前、术后X线平片分析植骨融合及脊柱后凸畸形矫正效果。结果术后随访14~43个月,平均24个月。患者手术切口均Ⅰ期愈合,结核无复发,均获骨性融合,融合时间3~7个月,平均4.5个月。无内固定松动、脱出及断裂。术前Cobb角30.6°,术后Cobb角17.6°,平均矫正13.0°。结论对胸腰椎结核患者行前路病灶清除后路椎弓根固定治疗可有效矫正脊柱后凸畸形,重建脊柱稳定性,获得良好的骨性融合。 Objective To summarize the clinical effect of anterior focal cleaning and posterior pedicle arcus vertebrae fixation on thoracolumbar spinal tuberculosis.Methods Thirty-one patients with thoracolumbar spinal tuberculosis were given anterior focal cleaning and posterior pedicle arcus vertebrae fixation from March 2005 to August 2007.Radiographs were analyzed to determine bony fusion and the correction of spinal kyphosis before and after operation.Results The follow-up lasted 14~43 months,with an average of 24 months.In all patients,phase I healing of surgical incisions was noted,with bony fusion,which needed 3~7 months averaging 4 5 months,and no tuberculosis relapsed.No internal fixation loosened,extruded or broken.The Cobb angle was 30.6°before surgery,17.6° after,with average correction of 23.0°.Conclusion Anterior focal cleaning and posterior pedicle arcus vertebrae fixation in thoracolumbar spinal tuberculosis can correct effectively kyphosis,reconstruct spinal stability and get good bony fusion.
