摘要
目的探讨医源性胆管损伤的诊断及治疗方法。方法采用影像学检查如CT与核磁检查可作为首选,以明确诊断、掌握病情,以确定采取何种术式及手术操作方法。结果胆道空肠(Roux-en-Y)吻合术9例,胆道破损修补术5例,胆道狭窄成形术4例,狭窄段胆管切除吻合术1例,胆道再吻合1例。经再次手术,全组均治愈。其中,1例术后9 d出现胆漏,充分引流后3周治愈,3例出现切口感染,经抗感染治疗后治愈。结论明确诊断,及早手术,选择正常的手术方式,术前充分准备,术后加强支持治疗,重建和修复胆肠的完整和通畅,是治疗胆管损伤的基本原则和方法。同时要注意预防,才能使胆管损伤的发病率降至更低。
