摘要
目的:研究儿童患者中类风湿性关节炎(rheumatoid arthritis,RA)和系统性红斑狼疮(systemic lulups erythematosus,SLE)血细胞的天然免疫功能状况,比较在儿童患者中这两种疾病红细胞表面CD35定量活性变化,探讨两种疾病的发病机制。方法:分离新鲜红细胞、淋巴细胞、粒细胞三系,分别测定其对肿瘤细胞的快速天然免疫黏附反应。应用流式细胞仪测定红细胞表面CD35的表达水平。结果:RA儿童患者红细胞、淋巴细胞以及粒细胞对肿瘤细胞黏附率均明显高于正常对照组(P<0.001);SLE儿童患者血细胞对肿瘤细胞黏附率又明显低于正常对照组(P<0.001);RA儿童患者的血细胞对肿瘤细胞黏附率,进展期最高,静止期其次,消退期再次,RA儿童患者各期均明显高于正常对照组(P<0.05)。RA儿童患者红细胞CD35明显高于正常对照组和SLE患者组(P<0.001);而SLE儿童患者CD35明显低于正常对照组(P<0.001);类风湿性关节炎红细胞CD35分子与红细胞黏附率呈显著正相关(γ=0.804,P<0.05)。结论:儿童SLE和RA患者红细胞天然免疫功能存在紊乱,并可能与病情的活动性有关。红细胞免疫功能检测可作为判定儿童患者RA与SLE病情演变及预后的指标之一。
