心肌标志物在诊断急性心肌损伤疾病中的临床应用 被引量:12

摘要 急性心肌损伤主要是由急性冠脉缺血造成,包括缺血性心肌损伤,如急性心肌梗死、手术后心肌梗死,不稳定心绞痛和非缺血性心肌损伤,如心肌炎、心脏创伤.急诊室及时可靠的检测出急性心肌缺血是诊断和进一步治疗的先决条件,只有10%~15%的有明显胸痛症状的急诊患者会发展为急性心肌梗死(AMI)[1].迄今为止,诊断急性心肌损伤的检验项目仍然使用血清中天冬氨酸氨基转移酶(AST)、肌酸激酶(CK)、乳酸脱氢酶(LD)及其同工酶的活性增高.酶活性检测的心肌敏感性和特异性都较差,特别是在ECG表现为小的无Q波的心肌梗死(MI)、不稳定心绞痛、心肌炎或中毒性心肌损伤以及伴有肾功能衰竭、多器官功能不全或伴骨骼肌损伤等疾病时难以准确诊断[2].显然,如果心肌损伤后血清中特异标志物增加是最理想的.
