电引导治疗新皮层癫痫大鼠的有效性研究

Efficacy of electric conduction on rats with penicillin-induced neocortical epilepsy

摘要 目的 探讨通过颅内-颅外电引导可以减少新皮层癫痫大鼠癫痫发作和继发性神经元损害,介绍一种治疗癫痫的新方法.方法 建立青霉素诱导的局灶性新皮层癫痫SD大鼠(n=60,4组,15只/组;即:空白对照组、引导组、假引导组、模型对照组)模型,采用专用引导电极建立引导模型.在麻醉状态下连续记录2h的脑电图(EEG),统计EEG癫痫放电次数,然后复苏进行2d的录像监测,统计行为学癫痫发作次数,最后取癫痫灶局部的皮层组织进行细胞凋亡率检测,应用统计学方法比较各组癫痫放电、行为学癫痫发作次数及细胞凋亡率.结果 模型组45只大鼠共成功建立癫痫模型41只,成功率为91.1%;引导组和假引导组各14只,模型对照组13只.引导组总EEG癫痫放电次数和扩散的EEG癫痫放电次数明显低于假引导组和模型对照组(P<0.01),但在局灶性EEG癫痫放电次数上三组比较无统计学差异(P=0.1344).引导组行为学癫痫发作次数低于模型对照组与假引导组(P<0.01).引导组的细胞凋亡率明显低于模型对照组与假引导组(P<0.01).结论 颅内-颅外电引导治疗大鼠局灶性新皮层癫痫有效,可明显减少EEG癫痫放电和行为学癫痫发作,并减少神经元凋亡率. Objective To confirm that conduction of the epileptiform discharge activity out of the cmnjum can by reduce the seizure and consequence neumn injury,and jntroduce the novel micm invasjve neurosurgical approach to epileptic thempy.Methods To build penicillin—induced neoconical epilepsy rat(n=60,4 groups,15 per group,including control group,conducting group,pseudo—conduction gmup and model control group).The specialized self-made conducting electl'ode was used to building conducting epileptic rat model.The epileptiform discharges were recorded by EEG with deep needle electrode for 2 hours under anesthesia,and seizures were monitored by video for 48 hours in waking.At last,the apoptosis ranio of neoconex was tested with now cytometer.Results we built 41(91.1%)penicillin—induced neocortical epiIepsy rats successfuUy.The mean frequency of total epileptifom discharges and frequency of diffused epileptiform discharges in EEG in conducting gmup were significantly HoweVer,signi6cant difference was not found in times of focal epileptifornl discharges among 3 test gmups.During video monitoring,significant difference presented in the ftequency of clinical seizure between conducting gmup and model contml gmup or pseudo-conducting group.less than the numbers in model contml gmup and pseudo—conduction gmup(P<0.01).Furthermore,apoptosis ratios of neuron in conducting group was significantly 1ess than the other two gmups(P<0.00 1).ConcIusions Conducting the epileptifom discharge activity out of the cranium can prevent the seizure and reduce epileptiform discharge and apoptosis mtio of neocortex in neocortical epilepsy rats.
