摘要
目的 研究探讨腹腔镜下内环口关闭术和传统开放式疝囊高位结扎术在小儿腹股沟疝治疗中的应用效果.方法回顾性分析本院2015年1月-2016年12月选择的200例小儿腹股沟疝患者,数字方法随机分为观察组(n=100)和对照组(n=100),其中观察组采用腹腔镜下内环口关闭术治疗,对照组采用传统开放式疝囊高位结扎术治疗.对两组患者的手术治疗效果、安全性等指标进行综合评价比较.结果(1)观察组手术治疗指标,均优于对照组,组间比较差异有统计学意义(P<0.05),提示腹腔镜疝修补术创伤性小,术后恢复快.(2)观察组患儿术后并发症发生率为4.00%(4/100),低于对照组并发症发生率为18.00%(18/100),组间比较差异有统计学意义(P<0.05),提示腹腔镜疝修补术安全性更好.(3)两组患儿随访1年期间,在腹股沟疼痛次数、腹股沟疝复发率比较差异无统计学意义(P>0.05),提示腹腔镜疝修补术能够达到开放式手术效果.结论腹腔镜下内环口关闭术和传统开放式疝囊高位结扎术均为治疗小儿腹股沟疝的有效治疗方式,但腹腔镜手术具有创伤性小、术后恢复快、治疗安全性好等优势,值得进一步推广应用.
Objective To investigate the effect of laparoscopic internal ring closure and traditional open sac high ligation in the treatment of inguinal hernia in children.Methods Retrospective analysis of 200 children with inguinal hernia selected in our hospital from January 2015 to December 2016.The numerical method was randomly divided into the observation group(N=100)and the control group(N=100).The observation group was under laparoscopic surgery.The ring was closed for treatment,and the control group was treated with traditional open sac high ligation.Comprehensive evaluation and comparison of the surgical treatment effect and safety of the two groups of patients.Results(1)The surgical treatment indexes of the observation group were better than that of the control group.The difference between the two groups was statistically significant(P<0.05),suggesting that the laparoscopic hernia repair is less traumatic and quicker.(2)The postoperative complication rate was 4.00%(4/100)in the observation group and 18.00%(18/100)in the control group.The difference between the groups was statistically significant(P<0.05),suggesting that laparoscopic hernia repair is safer.(3)There was no significant difference in the number of inguinal pain and inguinal hernia recurrence between the two groups during the follow-up period of 1 year(P>0.05),suggesting that laparoscopic hernia repair can achieve open surgery.Conclusion Laparoscopic internal ring closure and traditional open hernia sac high ligation are effective treatments for inguinal hernia in children,but laparoscopic surgery has the advantages of less traumatic,faster postoperative recovery and better treatment safety.It is worth further promotion and application.
