摘要
目的探讨血液灌流(HP)联合维持性血液透析(MHD)在慢性肾功能衰竭(CRF)患者中的应用效果.方法选取2019年8月—2021年6月我院收治的84例CRF患者为研究对象,按随机数字表法分为两组,各42例.对照组采用MHD治疗,试验组在对照组基础上采用HP治疗.对比两组治疗前后的肾功能指标、机体微炎症状态.结果治疗前,两组各项肾功能指标、机体微炎症状态比较,组间差异无统计学意义(P>0.05);治疗后,试验组血肌酐、血尿素氮分别为(320.01±24.88)μmol/L、(9.66±1.75)mmol/L,均低于对照组的(374.53±23.91)μmol/L、(12.53±1.68)mmol/L,肌酐清除率(37.88±3.51)mL/min高于对照组的(32.33±2.86)mL/min,组间差异有统计学意义(P<0.05);治疗后,试验组白细胞介素-6、超敏C反应蛋白、肿瘤坏死因子-α分别为(10.22±1.39)ng/L、(9.06±1.10)mg/L、(20.42±3.13)ng/L,均低于对照组的(13.66±1.27)ng/L、(12.59±1.47)mg/L、(26.74±3.26)ng/L,组间差异有统计学意义(P<0.05).结论HP联合MHD治疗CRF的临床效果确切,能够有效改善患者的肾功能,降低炎症指标水平,提高治疗效果.
Objective To explore the application effect of hemoperfusio(HP)combined with maintenance hemodialysis(MHD)in patients with chronic renal faiur(CRF).Methods A total of 84 CRF patients admitted to our hospital from August 2019 to June 2021 were selected as the research objects,and divided into two groups according to the random number table method,with 42 cases in each group.The control group was treated with MHD,and the experimental group was treated with HP on the basis of the control group.The renal function indexes and the micro-inflammatory state of the body were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference between the two groups in various renal function indexes and microinflammatory state(P>0.05);after treatment,serum creatinine(Scr)and blood urea nitrogen(BUN)in the experimental group were(320.01±24.88)μmol/L,(9.66±1.75)mmol/L,respectively,which were lower than(374.53±23.91)μmol/L,(12.53±1.68)mmol/L of the control group,the creatinine clearance(Ccr)was(37.88±3.51)mL/min,which was higher than(32.33±2.86)mL/min of the control group,and the differences between the groups were statistically significant(P<0.05);after treatment,interleukin-6,highsensitivity C-reactive protein,tumor necrosis factor-αin the experimental group were(10.22±1.39)ng/L,(9.06±1.10)mg/L,and(20.42±3.13)ng/L,respectively,which were all lower than(13.66±1.27)ng/L,(12.59±1.47)mg/L and(26.74±3.26)ng/L in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The clinical effect of HP combined with MHD in the treatment of CRF is definite,which can effectively improve the renal function of patients,reduce the level of inflammatory indicators,and improve the therapeutic effect.
