生物反馈电刺激联合盆底肌训练在脊髓损伤后神经源性膀胱患者中的临床应用效果

Clinical Application Effect of Biofeedback Electrical Stimulation Combined with Pelvic Floor Muscle Training in Patients with Neurogenic Bladder after Spinal Cord Injury

摘要 目的分析在脊髓损伤后神经源性膀胱患者康复治疗中采用生物反馈电刺激结合盆底肌训练的临床实际应用效果.方法将我院于2020年5月—2022年5月接受治疗的46例脊髓损伤后神经源性膀胱患者作为对象,按照随机数字表法分成两组,各23例.对照组采用盆底肌训练,观察组在对照组基础上联合生物反馈电刺激疗法.比较两组治疗前后的尿动力学水平、排尿功能、并发症发生情况.结果治疗前,两组膀胱容量、最大尿流率、膀胱压力、残余尿量比较,组间差异无统计学意义(P>0.05);治疗后,观察组的膀胱容量、膀胱压力均大于对照组,最大尿流率高于对照组,组间差异有统计学意义(P<0.05).治疗前,两组日平均单次排尿量、日平均排尿次数、日平均尿失禁次数比较,组间差异无统计学意义(P>0.05);治疗后,观察组日平均单次排尿量、日平均排尿次数均多于对照组,日平均尿失禁次数少于对照组,组间差异有统计学意义(P<0.05).观察组并发症总发生率4.35%低于对照组的26.09%,差异有统计学意义(P<0.05).结论针对脊髓损伤后神经源性膀胱患者实施生物反馈电刺激联合盆底肌训练可提升其排尿功能和尿动力学水平,减少并发症的发生,安全性较高,值得临床推广应用. Objective To analyze the clinical application effect of biofeedback electrical stimulation combined with pelvic floor muscle training in the rehabilitation of patients with neurogenic bladder after spinal cord injury.Methods A total of 46 patients with neurogenic bladder after spinal cord injury who received treatment in the hospital from May 2020 to May 2022 were divided into two groups according to random number table method,with 23 cases in each group.The control group received pelvic floor muscle training,and the observation group received combined biofeedback electrical stimulation therapy on the basis of the control group.The levels of urodynamics,urination function and complications before and after treatment were compared between the two groups.Results Before treatment,there were no significant differences in bladder volume,maximum urine flow rate,bladder pressure and residual urine volume between the two groups(P>0.05);after treatment,the bladder volume and bladder pressure of the observation group were greater than those of the control group,and the maximum urine flow rate was higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences in the daily average single urination volume,daily average number of urination and daily average number of incontinence between the two groups(P>0.05);after treatment,the average daily single urination volume and the average daily urination times in the observation group were higher than those in the control group,while the average daily incontinence times was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05).The total incidence of complications in the observation group was 4.35%lower than 26.09%in the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with neurogenic bladder after spinal cord injury,biofeedback electrical stimulation combined with pelvic floor muscle training can improve their urination function and urodynamics level,reduce the occurrence of complications,and is safe and worthy of clinical application.
