摘要
目的探讨等速离心训练联合远红外线照射对膝骨性关节炎(KOA)患者关节疼痛及功能康复的影响.方法选取2020年3月—2022年5月南京医科大学附属南京医院康复医学科收治的KOA患者78例,按随机数字表法分为两组,每组39例.对照组进行等速离心训练,观察组在此基础上进行远红外线照射治疗,均持续干预4周.对比两组的关节疼痛程度、膝关节功能、膝关节活动度及生活质量.结果观察组干预后的疼痛视觉模拟评分为(1.78±0.39)分,低于对照组的(3.32±0.46)分,膝关节活动度为(92.90±7.41)°,大于对照组的(82.78±7.71)°,组间差异有统计学意义(P<0.05).观察组干预后西安大略和麦克马斯特大学骨关节炎指数中的疼痛、僵硬及关节功能维度评分分别为(6.02±1.04)分、(1.92±0.39)分、(22.05±2.71)分,均低于对照组的(7.98±1.45)分、(2.91±0.48)分、(28.26±3.43)分,组间差异有统计学意义(P<0.05).观察组干预后生活质量综合评定问卷中的心理、躯体、社会功能及物质生活维度评分分别为(82.86±7.11)分、(81.89±6.45)分、(80.80±6.25)分、(81.86±5.30)分,均高于对照组的(70.68±6.05)分、(69.98±6.23)分、(70.71±5.17)分、(71.75±5.22)分,组间差异有统计学意义(P<0.05).结论等速离心训练联合远红外线照射能够减轻KOA患者的关节疼痛,增加膝关节活动度,促进膝关节功能恢复,提升生活质量.
Objective To investigate the effects of isokinetic eccentric training combined with far infrared radiation on joint pain and functional rehabilitation of patients with knee osteoarthritis(KOA).Methods A total of 78 patients with KOA admitted to the Department of Rehabilitation Medicine,Nanjing Hospital Affiliated to Nanjing Medical University from March 2020 to May 2022 were selected and were divided into two groups according to random number table method,with 39 cases in each group.The control group received isokinetic eccentric training,and the observation group received far infrared radiation treatment on this basis.Both groups continued to intervene for 4 weeks.The degree of joint pain,knee function,knee motion and quality of life were compared between the two groups.Results The pain visual analogue scale score of the observation group was(1.78±0.39)points,which was lower than(3.32±0.46)points of the control group,and the range of motion of the knee was(92.90±7.41)°,which was greater than(82.78±7.71)°of the control group,the differences between the two groups were statistically significant(P<0.05).The pain,stiffness and joint functional scores of the Western Ontario and McMaster Universities osteoarthritis index of the observation group after intervention were(6.02±1.04)points,(1.92±0.39)points and(22.05±2.71)points,respectively,which were lower than(7.98±1.45)points,(2.91±0.48)points and(28.26±3.43)points of the control group,and the differences between groups were statistically significant(P<0.05).The scores of psychological,physical,social function and material life dimension in the generic quality of life inventory-74 of the observation groups after intervention were(82.86±7.11)points,(81.89±6.45)points,(80.80±6.25)points,(81.86±5.30)points,were higher than(70.68±6.05)points,(69.98±6.23)points,(70.71±5.17)points,(71.75±5.22)points of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Isokinetic eccentric training combined with far infrared irradiation can alleviate joint pain,increase knee motion,promote knee function recovery and improve quality of life in patients with KOA.
