摘要
目的分析白内障超声乳化术联合房角分离术治疗闭角型青光眼(ACG)并发白内障的效果.方法选取2020年5月—2022年5月本院收治的78例ACG并发白内障患者,按随机数字表法分为两组,每组39例.对照组行白内障超声乳化术,观察组行白内障超声乳化术联合房角分离术,均观察至术后3个月.对比两组的眼压、视力恢复情况、眼部血流动力学及并发症发生情况.结果术后3个月,观察组的眼压为(13.69±1.20)mmHg,低于对照组的(17.36±1.53)mmHg,差异有统计学意义(P<0.05).术后3个月,观察组的最佳矫正视力、舒张末期血流速度、收缩期峰值血流速度分别为(0.64±0.09)、(9.78±2.03)cm/s、(32.94±3.75)cm/s,均优于对照组的(0.49±0.06)、(8.67±1.84)cm/s、(29.87±3.56)cm/s,组间差异有统计学意义(P<0.05).两组的并发症发生率比较,差异无统计学意义(P>0.05).结论采用白内障超声乳化术联合房角分离术治疗ACG并发白内障,可降低患者眼压,提高其视力水平,改善眼部血流动力学,且无严重并发症,临床应用价值较高.
Objective To analyze the effect of phacoemulsification combined with atrial angle separation in the treatment of atrial angle closure glaucoma(ACG)complicated cataract.Methods 78 patients with ACG complicated cataract admitted to our hospital from May 2020 to May 2022 were selected and were divided into two groups according to random number table method,with 39 cases in each group.The control group underwent phacoemulsification for cataract,and the observation group underwent phacoemulsification combined with atrial angle separation for cataract.Both groups were observed until 3 months after surgery.The intraocular pressure,visual acuity recovery,ocular hemodynamics and complications were compared betweenthetwo groups.Results 3 months afteroperation,the intraocular pressure ofthe observation group was(13.69±1.20)mmHg,lower than(17.36±1.53)mmHg of control group,the difference was statistically significant(P<0.05).3 months after operation,the best corrected visual acuity,end-diastolic blood flow velocity and peak systolic blood flow velocity in the observation group were(0.64±0.09),(9.78±2.03)cm/s and(32.94±3.75)cm/s,respectively,which were better than(0.49±0.06),(8.67±1.84)cm/s and(29.87±3.56)cm/s in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The combination of phacoemulsification and atrial angle separation in the treatment of ACG complicated cataract can reduce intraocular pressure,improve visual acuity and improve ocular hemodynamics without serious complications,which is of high clinical value.
